I understand that dance, by its nature, is a strenuous physical activity and carries with it the risk of injury. I understand that it is advisable to check with a physician before participating in dance or any strenuous activity. With those understandings, I release Broomfield Dance, Inc. (dba: Dance Arts Studio), any and all dance teachers teaching at Dance Arts Studio, and/or any student helpers under their direction from any and all liability should my dancer be injured in any way while attending Dance Arts Studio and its related activities.
I approve of Dance Arts Studio using any image, in its unaltered state, taken of my child to be used for advertising purposes in any media form.
PERMISSION FOR MEDICAL TREATMENT
In the event that an injury occurs and I cannot be reached, I consent to emergency medical treatment of my dancer.
Dance Arts Studio Policies relating to COVID19 Dance Arts Studio will follow the Colorado state regulations regarding the safety of our dancers during the COVID19 pandemic as well as any regulations our Board of Directors deem necessary at Dance Arts Studio.
Your part in keeping our dancers and staff safe...
Masks! Dancers will be required to wear a mask the entire time they are in the studio. This excludes only our Creative Movement classes for our youngest dancers. This policy will stay in place even if the state mandate is lifted. The reason behind this is that we cannot assure that all dancers will be 6 feet apart at all times while attending class at DAS.
Travel – If your dancer travels anywhere outside of Colorado, by any means of transportation, they will be required to quarantine for 14 days from the last date of travel from in-person classes at DAS. While in quarantine, they will have the option to attend class via Zoom. If they do not attend class via Zoom, they will be marked absent. Zoom links for each class will be available from your parent portal. No Parents will be allowed in the studio.
Exposure events – If your family attends an event (wedding, funeral, any celebration, etc) where you feel you have experienced a higher than normal risk for contracting COVID, Please self-quarantine from DAS in-person classes for 14 days.
Sickness – If your dancer, or someone in your household is known to have been exposed to a person who has COVID, your dancer should quarantine for 14 days from DAS in-person classes. If your dancer or anyone in your immediate family tests positive for COVID, contact the studio immediately! Your dancer will be required to quarantine from in-person classes at DAS for 14 days from exposure and cannot return to class in person until the earliest of 14 days OR she/he has a negative COVID test.
Our part in keeping our dancers and staff safe...
The cleaning of common areas in the studio. Taking no-touch temperatures of everyone entering DAS. Limiting the amount of extra people in the studio. (No parents will be allowed) Encouraging hand washing before and after each class.
Social Distancing – while we will encourage social distancing, we cannot assure that dancers will always be 6 feet apart during class, or waiting in the studio before or after class.
Zoom – All classes will be available in person or by Zoom. If there comes a time that you simply feel “safer at home”, your dancer will always have the option of Zoom. Zoom classes will be billed at the same rate as in-person classes. Links will be available for each class through your parent portal.
• The water fountain at the studio will not be available. Please be sure your dancer brings enough water for the duration of their classes.
• As we realize that dancing in a mask is hot, we will give the dancers a “breathing” break to step outside when possible. We will also keep the studio at a cooler than normal temperature.

Assumption of the Risk and Waiver of Liability Relating to CORONAVIRUS/COVID-19/Dance Arts Studio
The novel coronavirus, COVID-19 has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to- person contact. As a result, federal, state and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Dance Arts Studio (DAS) has put in place preventative measures to reduce the spread of COVID-19; however, DAS cannot guarantee that your child will not become infected with COVID-19.
I understand and will explain to my child/student at DAS the importance of complying with instructions and safeguards put in place by DAS to protect against the COVID-19 virus and its spread.
By accepting this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child may be exposed to, or infected by COVID-19 by attending DAS and that such exposure or infection may result in personal injury, illness or worse.
I understand that the risk of becoming exposed to or infected by COVID-19 at DAS may result from the actions of omissions of myself and others, including, but not limited to, DAS employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child including personal injury, illness, damage, loss, claim, liability, or expense of any kind, that my child may experience or incur in connection with my child's attendance at DAS or participation in DAS classes. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless DAS, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions or omissions of DAS, its employees, agents and representatives, whether a COVID-19 infection occurs before, during, or after participation in any DAS program, class or event.
Procedures & Policies for Fall Session 2020
Welcome to Dance Arts Studio! We are proud to offer professional quality classes and superior dance training at all levels. Each of our teachers holds a degree in Dance Education and/or Choreography. Our ten-month season offers your student dance education in a fun, cultural and artistic environment. There are two performances during this session. Our Holiday Showcase in December and we conclude or 10 month Season with a professional dance recital in early June giving your accomplished dancer an incredible performance opportunity.

This Dance Season starts Monday, August 17, 2020 and will end on approx. June 12, 2021, depending on the recital date we secure. The confirmed date will be announced in late fall.
There is a $45 Registration fee per family. This fee is non-refundable and is charged to your payment method upon registration, along with your first month’s tuition.

You will create a parent portal, choose your student’s classes and enroll in autopay for convenient, no worry, payments.
Tuition is due on the 15th of each month. Your method of payment on file will be automatically charged the full balance on your account on the 15th of each month. If your payment is declined for any reason, you will be contacted by DAS. From the contact date, you will have 5 business days to remedy the payment.
There is a $25 recovery fee if your account is not paid within the allotted time frame and your dancer will be suspended from class until a successful payment is made.
The studio will not be responsible for any cash payments made without obtaining a written receipt.
Any account that requires collection will also be responsible for court costs and attorney fees. If, for any reason, your dancer drops out of their class after March 15, 2021, your payment method on file will be charged $50 per class dropped and be subjected to additional choreography fees determined by DAS and your recital fee will not be refunded.
Students may not chew gum in class.
Student should not wear jewelry in class, and hair should be pulled back and out of their face.
Dance Arts is not responsible for lost or stolen items.
Absolutely no food or drink (except bottled water) will be allowed on the dance floor.
The lobby and dressing room areas need to be kept quiet and clean by students and parents.
Students must remain inside the reception area until a parent picks them up.
We depend on you for word of mouth advertising! For every dancer that you refer to our studio, you will receive
a $50 credit towards your final bill in May. The dancer must be enrolled by November 15th and stay through the
spring recital in June. Thank you in advance for inviting your friends to come to Dance Arts Studio!
The following requirements are mandatory and will be enforced by the dance teachers:
Hair must be tied up and away from face. Shoes should be labeled with dancer’s name. No t-shirts, baggy clothes, street clothes, sweats or pajama pants.
Ballet: Leotards, tights and pink ballet shoes. Younger dancers may wear skirts.
Ballet II and above: Must wear a black leotard and white or light pink tights. No skirts and no dance shorts.
Tap and Jazz: Any color leotard and tights, leggings, dance pants or dance shorts. All shoes must be black.
Modern/Lyrical: Any color leotard and tights, dance shorts, leggings or dance pants without feet.
Hip Hop: Any color or style of DANCEWEAR (must include a leotard) and black Hip Hop Shoes,
available to order from the studio.
Absolutely NO STREET SHOES WILL BE ALLOWED ON THE DANCE FLOOR.
Students not wearing appropriate dance attire will be asked to sit out and observe class.
Dance shoes, leotards, tights, skirts, dance shorts and jazz pants are available to order through the studio.
You must try on shoes and dancewear for sizing in the studio lobby. All merchandise ordered is non-returnable.
Labor Day: Monday, Sept. 7, 2020 (DAS is closed)
Thanksgiving: Nov. 23-27, 2020 (classes resume Mon. Nov 30)
Christmas: Dec 21 – Jan. 3, 2020 (classes resume Mon. Jan 4)
Spring Break: March 22– March 26, 2021 (classes resume Mon. Mar 29)
Memorial Day: Monday, May 31, 2021 (DAS is closed)
* Many schools have several “no school” days for professional development throughout the year. DAS still holds classes on these days.
We have requested our recital be on Saturday, June 12, 2021. We should have confirmation of the date in late fall.
A recital fee of $50 per family will be due in late March and is non-refundable. Costumes must be paid for before the end of January 2021.
We anticipate costume costs to be $50-$75 per class and are non-refundable once the order is placed.
All dancers have the option to not be in the recital.
All classes except “Turns, Leaps & Extensions” will perform in the recital.
Dropping, Adding , or Changing Classes/ Cancellation Policy
To ensure proper billing and class enrollment, it is your responsibility to manage your dancer’s needs to add, drop, or change a class. These tasks are easily managed from your parent portal. If your dancer needs to drop a class, you will need to drop them on your parent portal before the 14th of the month to avoid being charged for the class.
Any class dropped after the 10th of the month will be charged the full month's tuition for the next month. The program will prorate your class according to the date it is dropped or added online. Dance Arts Dance Arts reserves the right to cancel any class due to low enrollment. Fall Registration Fees and Summer Fees are non-refundable.
Classes missed for any reason must be made up in another class within a month of absence.
No refunds will be given for missed classes.
Students who are frequently absent or late may be asked to schedule private lessons at the dancer’s expense, in order to learn missed recital choreography.
Classes cancelled due to weather will not be rescheduled. Your dancer may attend any other class that is their level to make this up.
