2019-20 REGISTRATION WAIVER
Registration represents a financial commitment through the end of the school year June 2020. Tuition is based on the dance school year September through June regardless of the starting date and NOT by the number of classes taken. To register and reserve space in class, the following is due at registration: $25. Registration fee and non-refundable first tuition payment with completed registration form.
All payments are due the first class of the month. Electronic payments will be deducted on the 5th of each month. We accept Visa, Master, Discover credit cards, debit cards and cash as methods of payment. NO PERSONAL CHECKS, No AMEX. All payment’s received after the 15th of the month are subject to a $25 late fee per payment. If classes are dropped there are no refunds or credits, payments are non-transferable. If payment is not received by the 15th of the month, the student may be asked to sit and observe. We reserve the right to terminate a student’s enrollment if payments are not current.
CLASSES AND CLASS PLACEMENT
Students are placed into classes based on age, ability, experience and maturity. Class placement and advancement is at the discretion of the director and teachers. We reserve the right to cancel, reschedule or combine classes with inadequate enrollment. We apologize in advance for any inconvenience.
Any student who is a disciplinary problem in class and affects the learning of all may be removed from class. Parents will be notified and a conference may be requested. Parents wishing to discuss their students’ behavior or progress should request this in the office.
The dress code/uniform was designed for flexibility, practicality, uniformity and discipline. It allows the teacher to evaluate and correct student alignment and placement. Therefore students are required to have the proper class attire. School uniform is available on site at dk boutique. Students who come to class out of uniform will be asked to sit and observe. All dancers must wear cover-ups when leaving Dance School. Do not wear dance shoes outdoors.
Emails and newsletters will be distributed throughout the year. If you are not getting our monthly emails, please let the office know. Please check your student’s dance bags for any newsletters as well as the MPSD bulletin board and our website, Facebook, and Instagram pages for updates, events and schedule changes.
We observe the following holidays: Thanksgiving Day and the Friday after, Winter Break (Christmas – New Year’s), MLK Birthday, Spring Break (Easter week), Memorial Day Weekend (Friday-Monday). The full MPSD calendar is available on the MPSD website and in the dance school office.
Regular attendance in assigned classes is a must. Payment adjustments are not made for absences due to illness, vacation or any other reason. Space is reserved for the student whether present or not and the cost of operation is continuous. Please notify MPSD if you must miss a class. Make-up classes may be scheduled provided there is space in available in a comparable class. No make-ups after February 1st. Excessive absenteeism may result in my child being ineligible to participate in MPSD recital, performances and competitions. Please arrive at least 10 minutes before class. Students who arrive late for class run the risk of injuring themselves by not being properly warmed-up. They may be asked to sit and observe class. This is not a disciplinary measure, but is done with the student’s best interests in mind.
In case of inclement weather, text alerts will be send 2-hrs prior to studio opening hours. In addition, messages will be posted on our website and social media pages. The safety and well-being of our students, parents and staff is always our priority.
For the safety of your children, please have an adult enter the building to pick up any children under the age of 13 or in any classes ending after 8pm. Please be on time to pick-up your child after class as a courtesy to your child and the MPSD staff. MPSD is not responsible for children outside of the class room. Late pick-up fee of $15. per 15 minute intervals, beginning 10 minutes after class ends will be added to the account of students with excessive late pick-up.
MPSD is not responsible for lost, misplaced, stolen or damaged items. MPSD is also not responsible for items not claimed by 6/16 including but not limited to 2019: Trophies, Recital Photos, DVD’s T-shirts, Costumes and/or accessories,
I hereby agree to let my child participate in the Maria Priadka School of Dance, Inc. I understand that there are certain risks of injury inherent in dancing, as well as in traveling and other related activities incidental to my child’s participation, and I am willing to assume these risks. I hereby certify that my child is fully capable of participation in the MPSD, Inc. activities and that he/she is medically authorized and healthy and has no physical or mental disabilities or infirmities that would restrict full participation. I hereby authorize the Director of MPSD, Inc. to act for me according to her best judgment in any emergency requiring medical attention if I cannot be reached. In addition to giving full consent for my child’s participation, I hereby waive, release, indemnify and hold harmless the Maria Priadka School of Dance Inc. its employees, teachers, sponsors, partners, supervisors and representative for any injuries that may be suffered by my child in the normal course of participation in MPSD, Inc activities incidental thereto, whether the result of negligence or any other cause. The Maria Priadka School of Dance, Inc is not responsible for any lost, misplaced or damaged property. I also recognize The Maria Priadka School of Dance, Inc is not responsible for students when they are outside of the classroom and it is my responsibility to pick up my child at the designated end of class.
I hereby register my child at the Maria Priadka School of Dance Inc. for the 2019-2020 Dance School year ending on Recital Day 2020. I have read, understand, and have received a copy of all the policies, terms and conditions as stated above and attached hereto, including but not limited to tuition rates and payments, costume payments, dress code, class placement, general recital information rules and regulations as posted.
The Maria Priadka School of Dance, Inc reserves the rights to any video or photographs of students and student performances for promotional use with no fees for said use.
