You must be over 18 to register for an account and/or the Legal Guardian of the dancer you are registering

I/We understand that there are numerous risks associated with participating in dance activities, including those risks present during classes and activities, on the premises before, during, and after classes, as well as in travel to and from the studio and various venues for performances. Some of the specific risks include the placement of unusual stresses on the body, falls, and tripping, which may lead to accidents resulting in, but not limited to, mild to severe bodily injury. I/We understand that the studio cannot be responsible for any injuries or damages experienced by my.our child during his/her participation in such activities.

Absences:
Each dancer is allowed up to three (3) unexcused absences from classes and rehearsals per semester.
*Excused absences receive prior approval from the director; advance notice of absence does not mean it is excused.

The following are considered “excused” absences:
ï‚· Death in the family
ï‚· Scheduled school programs or performances required for credit
ï‚· Wedding or religious dedication of you or a member of your family
ï‚· If you are seriously ill or infectious

**Dancers will watch class/rehearsal in the case of injury or mild illness.

You will NOT be excused for school dances, day dates, etc.

Punctuality is just as important as attendance, please be on time and prepared for class. Two (2) occurrences of tardiness will count as an unexcused absence.
Dancers who exceed 3 absences per semester are subject to being removed from missed choreography. Dancers who exceed 5 absence per semester are subject to being dismissed from their choreography class / Tour Team.

I agree that I, as well as my dancer, are personally responsible for my/our safety and actions while in Jolie Arts Dance Center. I agree to comply with all JADC policies and rules, including but not limited to all JADC policies, guidelines, signage, and instructions. Guidelines including washing hands before and after classes, agreeing to having temperature taken prior to participation in any and all activities at JADC, social distancing before, during, and after classes, wearing a mask encouraged but not required, and if I/We or my/our child is experiencing any symptoms of fever, cough, shortness of breath, nausea, diarrhea, congestion we will stay home and request online participation.

By Signing the Agreement and Releases, I/We consent and give permission to JADC, and those acting its authority, to use the name and likeness of my child for the purpose of marketing JADC to the media.

In an emergency situation, when parental permission is not available, I/We hereby grant and give my/our permission for a staff member at the studio to seek emergency medical treatment for my/our child. In my/our absence or inability to communicate with emergency or hospital personnel, I/We hereby grant the studio authority to release, for the purposes of providing medical treatment, my/our child to the care of medical personnel or physicians as the studio determines as reasonably appropriate.

Tuition:
I agree Jolie Arts Dance Center will auto-pull my tuition on the 1st of every month. If my card declines I have until the 10th to make my balance $0 and a $25 late fee will be added. If my account is delinquent for 2 consecutive months I understand I may be dismissed from Jolie Arts Dance Center unless an arrangement has been made with Leeza.

Monthly Rate- Per Dancer based on hours enrolled for a week + $15 costume/performance fee evenly distributed. 
Total Costume/Performance fees amount for the dance season: $120

 

Courtesy and Respect:
All JADC Dancers will be held to the highest standards of conduct. Communicating negatively about anyone in or outside our studio will not be tolerated. Dancers are expected to show courtesy and respect to all instructors and other students. Any student who exhibits repeated negative or disrespectful behavior may be asked to leave the studio. JADC Dancers are always respectful, supportive, encouraging and are positive in spirit, attitude and behavior. We want to create a safe and creative space for all our dancers at all times.

Cancellation of Classes:
Please let us know by the 15th of the month BEFORE you plan to cancel so we can make sure auto-pay doesn't pull for the following month.

By agreeing I/We consent and give permission to the studio, and those acting under its authority, to use photos and videos of my child in class, from photo shoots, and performances for the purposes of marketing the studio to the media. I understand my child's name will not be released.

I/We have read and clearly understand the terms of all the waivers and agreements. I/We give permission for my/our child to participate in all studio activities, including classes, photo shoots, performances, etc. I/We also release and hold harmless the studio, its officers, directors, agents, employees, and those acting under its authority from all actions, claims, and liabilities relating to my/our child's participation in any and all programs.

I further agree to identify and hold harmless Leeza Fullmer, Jolie Arts Dance Center, it's teachers, officers, agents, and employees from injuries, damages and losses sustained by me or my child arising out of, connected with, or in any way associated with the activities of the program.
