Liability Waiver
I/We realize that participation in dance class and actives could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by encore performing arts studios llc.
I/We agree to release and hold harmless encore performance studios llc, including its teachers, dancers, staff member and facilities use by both entities from any cause of action, claims, or demands, now and in the future. I/we will not hold encore performance studios llc liable for any personal injury or any personal property damage which may occur on the premises before, during or after classes, workshops, rehearsals, recital or special events. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to an damage/we may cause to the facility utilized by encore performance studies llc.
In the event that I/we should observe any unsafe conducts or conditions before, during, or after classes, I/we agree to report the unsafe conduct or conditions to the instruct or staff member as soon as possible.
We also release all photography and videography of myself or my students taken during classes and events to be used at the discretion of Encore for advertising and media.
If my student is sick, they will not attend class. By attending I attest that to my knowledge my child does not have any symptoms of fever, vomiting, or sore throat, or shortness of breathe and has not been exposed to anyone who has tested positive for Covid-19 within the appropriate time frame.
We understand we are held responsible for all charges and fees occurred during our time at Encore, abiding to the stipulations in the parent handbook for pricing, fees, processing fees, and any deadlines for class drops and recital opt-outs.
