You must be over 18 years to register for an account.

I hereby grant permission for the above mentioned child/children to participate in the Denise Lester Dance Academy dance classes.

I hereby release the Denise Lester Dance Academy and all staff members from all claims and damages arising from participation of my child in such activities whether it is in class, recital, exams, showcases, or competition for in studio or online virtual classes.

In the event of unforeseen circumstances resulting in in studio classes not being mandated by Government of Canada, we will automatically resume online virtual classes. Virtual Classes would replace in studio class sessions at this point.
Once a session has begun, classes are non-refundable for that session.

As the studio has taken all precautionary measures set forth by Public Health for COVID -19 by choosing to participate in classes you release the studio of any unforeseen negative events such as, but not limited too the infection of the COVID-19 virus.

Pictures and video of the students may be used on our web site, on our face book or Instagram page or for other forms of advertising for the studio.

It is the parent’s responsibility to make their child understand that pictures/video taken by students, parents or anyone not employed by the studio may NOT be posted on social media without the expressed permission from Denise Lester Dance Academy and the person(s) in the picture or video. Please refer to “Terms and Conditions” on the website for further info.

The balance for costumes is the responsibility of the parents to pay once the non-refundable costume deposit has been paid. If you take part in competition, the competition entrance fees are non-refundable.

By providing my email address, I consent to email communication with the Denise Lester Dance Academy. If I choose to opt out of receiving emails I know that I will not be receiving important class and studio information and will be missing important updates. It will be my responsibility to come into the studio to find out what emails and information I have missed.
