My child /myself has permission to participate in the Dance with Miss Andrea program. I waive any right to claim against DWMA, Andrea Hall or DWMA staff in event of an accident, injury, or loss of personal property. I grant my permission for my child’s photo to be used in promotional materials or on the Dance with Miss Andrea Facebook page, taken for positive public relations purposes. I have read the aforementioned policies and procedures.

Tuition Policy: I agree to pay tuition in advance for my child's enrollment. Students whose accounts are arrears for any reason by the end of the 2nd class will not be able to participate and no adjustments will be made for missed classes. Tuition does not change with school closings or holidays. The month of May is the only month pro-rated. Tuition is due on the 1st of every month. Payments made after the 8th of the month are considered late and are subject to a $10.00 late fee. You will receive a statement monthly statement through email.
Refund Policy: All tuition is to be paid in advance of student participation. All Registration fees are Non-Refundable. No tuition refunds will be issued unless a student has dropped before attending first class and issued a withdrawal in writing. All Recital fees are Non-Refundable.

Inclement Weather Policy: In the event of inclement weather, DWMA will make every effort to schedule make ups for canceled classes.

Hair and Attire Policy: Dancers will wear long hair in a bun or ponytail. Dancers will come to class dressed in a leotard, tights and proper shoes, unless approved by instructor. There is a limited inventory of dance shoes to borrow at the studio, if needed
