12-Month Enrollment: By completing this registration, you are committing to a 12-month contract, except for camps/workshops with set dates. This contract cannot be paused or transferred. Your tuition is billed monthly on the 1st for 12 consecutive months. Congratulations! You automatically save 20% off tuition this way!
Month-to-Month Enrollment: If you prefer a Month-to-Month tuition plan, DO NOT CONTINUE past this step. Contact our office at info@freedomdanceaz.com, or speak with our front desk receptionist, and we will complete your enrollment for you. We require 30-day notice to drop a class.
Unlimited Tuition Plans: If you want to be on one of our Unlimited Tuition Plans, DO NOT CONTINUE past this step until an admin has set your fixed tuition rate.
Registration: A non-refundable/non-transferable fee of $20 per student is required each dance year in July or upon enrollment.
Payment of Fees: Tuition must be paid by automatic bank account debit or automatic credit card payment. First month's tuition is due upon registration. Tuition will be debited from your bank account or charged to your credit card on the 1st day of each month. If your credit/debit card is unable to be processed on the 1st of the month, you will receive an email notification. If tuition is not paid by the 5th, a $10 late fee will be added to your account. If outstanding tuition is not paid by the 15th of the month, the student may be withdrawn from the school. Depending on the availability of classes, students may re-enroll once balance is $0. There are no refunds or credit transfers after tuition has been paid.
ESA Students using Class Wallet: Tuition will be paid on a quarterly basis with due dates on second Monday of July (after summer break), October 1st, January 1st, and April 1st to align with ESA's funding schedule. We understand the delays that are occurring with vendor payments through Class Wallet, so there will not be any late fees assessed for Class Wallet payments as long as we receive notification from ESA that a payment has been submitted. There are no refunds or credit transfers after tuition has been paid.
June Dance Recital, May Showcase, and Fees: Dance students have the opportunity to perform on stage in our June Recital! Tumbling, Cheer, and Acro students show off their skills in a May Showcase! These shows are optional, but participation is highly encouraged because performance is part of our education. We assume every student is participating in the show, unless you email us to opt out. Please pay attention to all due dates for show and costume fees due in the Fall, posted on our website. These will be auto-drafted on the due date with your card/ACH on file.
Costumes: I understand that if my child participates in the recital/showcase, there will be a $75 costume fee per class in addition to the recital/showcase fee.
Withdrawals and Refunds: Should it become necessary to discontinue classes for any reason, 30 days notice is required. Withdrawal must be done through the school office and not with the teacher. No withdrawals will be accepted between April 1, 2024 and June 1, 2024 for those in the showcase/recital. To withdraw from classes, a parent or adult student must: Inform studio administration and complete and sign a withdrawal form provided by the studio office. All automatic bank-debiting or credit card charges will stop after the one-month period. Freedom Danceworks reserves the right to terminate lessons to any student without notice. In such a case, a refund for unused lessons will be given.
Extreme Weather or Unexpected Interruption of classes: If the studio must cancel classes due to extreme weather or events beyond our control, such as power outages and pandemics, makeup classes will be offered. No refunds.
Substitutions: The school reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and the school cannot arrange a substitute, makeup classes will be offered.
Dress Code: Required dance wear and dance shoes must be worn to all classes. Failure to wear required dance wear to class could result in students being asked to sit out the class. Repeated failure to wear required dance wear could result in the termination of lessons. Students are not permitted to wear jeans or shoes worn on the street or outdoors to any class. (For hip hop, we recommend students wear sneakers designated just for class.) For safety reasons, dancers and tumblers are not allowed to wear any jewelry other than stud earrings. Long hair must be pulled away from the face. Dress Code requirements are in our information packet, available at the front desk.
Care of Students: The studio is not responsible for providing before or after class care for students. Students are not to be left at the studio for excessive time periods before or after their classes. All students should be escorted by a parent/guardian into the studio and brought to their appropriate classroom. All parents are responsible for making sure their child safely enters our studio, regardless of their age. They are NOT allowed to be dropped off at the curb with the assumption of FDX staff and/or faculty being present. Parents/Guardians are requested to be in the studio’s vicinity with 5 minutes of their child’s dismal from class. A courtesy call should be made if the parent/guardian of the student will be late, allowing the instructor and student to be notified in a timely manner. If you are more than 10 minutes late picking up your student, or they are dropped off more than 10 minutes prior to class, you may be subject to a $10 childcare fee. Our faculty is not paid beyond class time to babysit your student.
Parent’s Responsibility to be Aware of Dates and Events: It is the responsibility of the parent or adult student to be aware of all studio activities, such as fee due dates, recitals, extra classes, and dates the studio is open or closed. The studio will post all such notices on the website as well as emailing/sending notices home with the students and posting the information on social media, when applicable. It is the parent’s responsibility to regularly check emails to ensure they are informed. It is the responsibility of the parents or adult students to inform the studio of any address or telephone number changes.
Injuries: Parents/legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on studio property resulting from normal dance and tumbling activity or any other activity conducted by the students or parents before, during, or after class time.
Animals/Barns: Parents are responsible to watch their children on the property and around the animals. Here are some rules to keep everyone safe: Do not run by the horses, but calmly walk. Never touch any of the animals. Students should walk to/from the parking lot to the studio/tumbling barn and not linger or wander the property. Parents/legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on studio property or neighboring property resulting from breaking these rules. By completing this waiver, you agree to hold Freedom Danceworks, Hayden Acres, and all property owners harmless.
Photo Release: The school is hereby granted permission to take photographs of the students to use in brochures, websites, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name. If, for legal reasons, (i.e. foster children) your student's photograph may not be displayed, you must notify the studio in writing.
Attendance & Make-ups: Attending class on a regular basis is important in any learning experience. The consistency and repetition of training is necessary for proper technique to be learned. Arrive early to class. Students may not be allowed to take class if they are more than 15 minutes late. If a child misses a class and monthly tuition has been paid, the dancer is allowed one make-up class for each class missed. Make-up classes are a courtesy for registered students only and are not transferable. A make-up class is any other class that we offer for their age and dance level. Our teachers highly recommend that dancers take advantage of make-up classes in order to help them from falling behind in their dance training. Making the class up is the responsibility of the parent and student. You have the entire dance season to make-up any missed classes. If you elect not to make up the missed class, it will not be credited to your next month’s tuition. There are no transfers, refunds, or credits.
Class Placement: Class placement is determined by the instructor. Dance education requires “hands-on” instruction as well as verbal instruction. Instructors will regularly correct your child by touching their arms, legs, feet, hips, back, and head to move them to the correct position. Instructors will also verbally correct your child during the class and offer encouraging feedback.
Summer Camps & Workshops: Full tuition is due upon registration of camps/workshops. Refunds, less a 15% processing fee, will be given no later than 7 days prior.
Notice to Customer:
You are entitled to a copy of this contract. If you cancel the contract in the fifteen-day period, you are entitled to a refund of your money less payment of the registration fee and for dance lessons or services received before cancellation. Refunds must be made within ten days, not including Sundays and holidays of receipt of the cancellation notice by the studio.
