The undersigned parent/student indemnifies and agrees to hold harmless Valley Dance LLC, its Board of Directors, Officers, Instructors, Agents, and assigns from any and all liability whatsoever, for any damage or injuries, and from any and all claims and demands, including attorney fees, arising out of the party’s participation in both in person or online dance lessons, classes, workshops, performances, fundraisers and other related activities provided by Valley Dance.
The undersigned parent/student understands if you still have not paid the total amount payable for any bill after we send you a warning, we may refer your debt to a debt collection agency, and if we do so you must pay any costs that we incur in connection with the recovery of the unpaid bill (including the agency’s fees and any legal fees). We will not refer your debt to a debt collection agency if we have agreed to an installment plan and you make your payments.
The undersigned parent/student understands that students may occasionally appear in promotional performance videos, photos, brochures, and other materials as a result of his/her association with Valley Dance. By registering a child/student for class, such use of the child/student’s name and likeness are agreed to and acknowledged and, accordingly, all right, title and interest in same are waived.
I understand that in the event the studio must shut-down due to Covid-19, all classes will automatically be offered on Zoom until we are able to re-open. I understand tuition will stay the same and no refunds will be given in such circumstances.
I agree to pay all participation fees. I understand tuition will not be refunded for payments that were already processed and that absences will not be refunded. I understand that profits from fundraisers are non-refundable. If you withdraw from classes, any leftover credits/fundraiser profits in your account will stay in your account for one year. Â After that, if they are unused, they will go into a scholarship fund for dancers in need.Â
I understand that if I wish to withdraw my child from class, I must submit it in writing (email or letter) at least 2 weeks before the 1st of the month.
I have carefully read this Agreement, Waiver, Release & Assumption of Risk and fully understand its contents. I understand that this is an assumption of risk and release of liability, and I sign it of my own free will.
