I hereby give my permission for my child(ren) to participate in Moving Art programmes as indicated below. I hereby release Moving Art and Moving Art staff for all liability. I also give permission for pictures/videos of my child(ren) to be taken and used, with permission for Moving Art promotion. I further understand that the information on this form is collected for administration purposes only. I also give consent to receive emails from Moving Art and/or Carla Tucker. Withdrawing from a dance class will invoke a one month penalty payment (Triple Threat classes is a two month payment penalty). In the event of a withdraw, the office must be notified in writing 14 days prior to next scheduled payment date. For term classes, withdrawing within first 2 weeks of classes, students will receive a 50% refund and after the first 2 weeks, there will be no refund provided. For Summer Sessions a 75% refund is available until May, otherwise no refunds after May 30th. Payments must be received prior to class, otherwise class may be denied. There is a $25 nsf fee charged for any returned cheque. Minimum enrolment is required for classes to run. Moving Art is not responsible for lost or stolen items at any time.
