Waiver of Liability / Acknowledgment of Risk / Photo & Video Release
* I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold harmless Grace Dance Academy, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Grace Dance Academy liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes.
*I hereby release and agree to hold Grace Dance Academy harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Grace Dance Academy. I understand that this release discharges Grace Dance Academy from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Grace Dance Academy. This liability waiver and release extends to the studio together with all owners, partners, and employees.
*Furthermore, I understand that I should be aware of my, and/or my child’s physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.
*Further, I authorize and agree that Grace Dance Academy may take and use photographs/videos of me and/or my child for purposes of record keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same.
