HDS Ballet Policy

Dance Better. Live Better.

Preface:
Huang Dance Studio, established in 2012 by Artistic Director Junshuang Huang and Principal Cecilia Chen, is dedicated to providing exceptional ballet education. Our commitment to excellence comes with certain responsibilities that both students and parents must acknowledge.

Attendance:
Regular attendance is crucial for optimal learning.
Students may miss classes, but makeup classes within the paid semester are mandatory.
Excessive absences are discouraged, and no discounts are provided for missed classes.

Tuition, Payment, and Credits:
Tuition is non-negotiable, non-refundable, and non-transferable.
Payments are made on a semester basis through various methods.
Online transactions may involve a credit system for refunds, cancellations, etc.
Accounts will not be refunded or cashed out.
Balances must be used or spent before dropping off regular classes.
Once dropped off, the balance becomes unavailable.
Due to the weather conditions, we are following the school's decisions and canceling regular classes. There will be no credit, refund, or extra open class to make up for missed sessions.
The competition, exam, and recital registration fee is non-refundable under any circumstances.

Cancellation:
Withdrawing mid-term requires full payment of the term's fees.
Official withdrawal notification is necessary to avoid accumulating fees.

Enrollment:
Ballet classes are categorized by age and skill.
New students undergo audition try-outs for proper class placement.
Dress code adherence is mandatory during audition try-outs.
Students must not learn ballet from other schools to maintain the studio's teaching integrity.
Contemporary dance becomes mandatory for students up to Grade 3.

Dress Code:
Hair must be secured away from the face.
Proper attire aids in correcting body alignment and technique.
Label dancewear; the studio is not responsible for lost or stolen items.
Uniform compliance is essential for RAD classes.

Class Etiquette and Expectations:
Punctuality and appropriate attire are expected.
Disciplined behavior and no running, climbing, or horseplay allowed.
Gum, food, and drinks are prohibited.
Parents are not allowed in classrooms or to record dancers.
Phones must be silent or off during class.
Respectful behavior is mandatory; bullying is not tolerated.

Private Lessons:
Private lessons are scheduled by appointment.

Adult Classes:
Adult students need a dance account for class registration.
Dance cards are purchased instead of paying tuition.
Drop-ins are allowed if the class is not sold out.
Makeup classes are permitted within the current semester.
Dance cards are non-transferable, non-refundable, and not redeemable for cash.

Note: The policies outlined above are effective immediately. HDS Ballet reserves the right to make changes to these policies as needed.

I fully understand and acknowledge that participation in any dance class and other events/programs (such as workshops, rehearsals, performances, grading tests, etc.) associated with the dancing activities offered by Huang Dance Studio (or the "School") could involve a possibility of physical injury. Despite precautions, accidents and injuries may still occur. By signing this liability waiver form, I voluntarily agree, therefore, to assume all risks and responsibilities for any injuries or accidents that might occur to my child during any of the Huang Dance Studio classes, rehearsals, performances, or related activities. I also agree to exempt, release, and indemnify Huang Dance Studio, its owners, teachers, staff members, and facilities from any and all liability claims, demands, causes of action, and costs/expenses resulting from injuries, damages, and loss sustained by me or my child that arise out of, in connection with, or are in any way associated with the activities involved in the classes and/or programs/activities offered by Huang Dance Studio.

I understand that Huang Dance Studio is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio, and that Huang Dance Studio will only be supervising my child when he or she is participating in scheduled dance classes, activities, and programs.

I understand that Huang Dance Studio is not responsible for personal property that is lost, damaged, or stolen while I or my child is at or on Huang Dance Studio property.

I also fully understand that the tuition, once paid, is non-refundable and non-transferable. A $25 charge will be applied for all returned checks.

I further understand that Huang Dance Studio promotes dance as an art form not only to its students but also to the community at large. As such, articles, brochures, videos, and websites, including images of myself and/or my child, may be used in Huang Dance Studio's promotional materials (these materials may include but are not limited to photographs and videos of classes and performances, newsletters, and other promotional materials, and internet web pages). I hereby permit the use of my or my child's image to be photographed, videotaped, or otherwise recorded for use in Huang Dance Studio's promotional materials, and that I and my child have no rights to the same and will not be compensated for the same.

I have read this liability waiver form and have been given the opportunity to speak with the representative of Huang Dance Studio prior to signing this form. I agree to be bound by the terms set forth above.

(NOTE: In case of emergencies, the School will contact the student's parents/guardians provided in this form. If the parents/guardians cannot be reached, the School will call the emergency agent for immediate care for the students.)

This Studio Rental Agreement ("Agreement") is entered into as of Jan 1st, 2024, by and between Huang Dance Studio LLC, hereinafter referred to as "HDS Ballet," and the undersigned renter, hereinafter referred to as the "Renter."

Rentals:
Studio Space Intended Use: Movement-based activities (fitness, martial arts, dance, etc.)
Rental Rate: $45 per hour (10% discount for 10+ hours/week)
Scheduling: Regular weekly classes must confirm schedules one month in advance.

Payment:
Renters pay at or before each rental period.
Late fee: $15 per month for overdue accounts; $30 for returned checks.

Cancellation Policies:

Cancellation by Renter:
If the renter needs to cancel their private class for a specific date, HDS Ballet must be notified no later than 48 hours before the scheduled rental time.
Cancellation with less than 48 hours' notice may result in HDS Ballet retaining some or all of the rental monies, especially if the studio space could have been rented to another party.
If a cancellation is arranged after the rent has been paid, the remaining balance will be applied towards future rentals of HDS Ballet. No refunds will be provided.

Cancellation by HDS Ballet:
HDS Ballet reserves the right to cancel this rental agreement at any time.
In the event of cancellation by HDS Ballet, any prepaid rental fees will be fully refunded to the renter.

Conditions:
Smoking/Incense/Flames: Prohibited inside and on the back balcony.
Stereo: Available for rehearsals, classes, and performances.
Props: Must not damage the dance floor; proper padding required.
Storage: No leaving equipment or belongings in the studio.
Keys: Not to be copied or shared; must be returned promptly.
Security: Renters responsible for studio security during their period.
Emergencies: Call HDS Ballet Emergency Contact or 911 accordingly.
Damage: Renters assume full responsibility for any damage.
Liability: Renters assume liability for injuries to anyone they invite to the studio.
Housekeeping: Renters responsible for studio cleanliness before leaving.
Parking: All renters must park behind the plaza building.

Contracts:
Renters acknowledge this contract by utilizing the studio.

Future Policy Changes:
HDS Ballet reserves the right to change rental policies, with renters informed of any changes.

Release and Indemnity:
Renters release HDS Ballet from liability for damages or injuries caused during the use of the premises.
Renters agree to indemnify and hold HDS Ballet harmless from any responsibility or liability.

Acknowledgment:
By utilizing the studio, the Renter acknowledges the terms of this agreement, binding both parties.

Effective Date:
This agreement is effective as of Jan 1st,2024.
