COMMUNICABLE DISEASE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
In consideration of being allowed to participate in any way in the program, related events and activities, I the undersigned, acknowledge, appreciate, and agree that:
I am aware there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.
I acknowledge my responsibility not to attend class if I am exhibiting symptoms of COVID-19. If, however, I observe any symptoms during my participation or presence at a class or event, I will remove myself from participation, and bring such to the attention of my teacher or another adult associated with the school, immediately.
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Deborah MacKay Dance Inc., its officers, officials, agents and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct any event (RELEASEES), from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARENTS/GUARDIANS OF PARTICIPANT OF MINOR AGE (UNDER AGE 18 AT TIME OF REGISTRATION)
This is to certify that I, as parent/guardian with legal responsibility for this participant, do acknowledge my responsibility to restrict my child from play or practice if they exhibit any symptoms of COVID-19, and consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liability incidents to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.

1- Parents agree to keep their child home if they do not feel well, or have been exposed to Covid 19. That exposure includes the immediate family.
2- Notify school immediately if tested for Covid or you receive a positive test result.
3-Arrive in the parking lot 5 minutes before class ends so pick up runs smoothly
4- Masks at this point are suggested for unvaccinated but will not be enforced. Mask policy subject to change.
Waiting room policy pertaining to parents, as of 7-28-21.
Parents of students ages 3-6 will be allowed to bring their children into the waiting room before class. We are asking they don’t wait in the waiting room or come into pick up. We will bring your students out to you after class.
All other students should be dropped off and picked up with no parent entering the waiting room.
Enter the west side and exit the east side doors. We will continue to use our outside exits through studio A and B
Absence from class is not refundable but can be made up in a comparable class. There are no refunds once the first class has been taken. Once a class has been taken within a new term, payment is due in full for that term. Tuition is due the first day of each quarter and parent agrees to pay each quarter unless written withdrawal is received.
Class withdrawal: If dropping a class written e mail is required 2 weeks prior to the beginning of the next term.
All accounts (tuition, costume, any other dance related costs) must be paid before handing out costumes.
Participation in the school means you agree to pay for the costume in full, once they have been ordered. If student does not participate in the show or withdraws from the school, parent is still responsible for all costume costs. The costume companies do not refund me, and I pay up front and in full to get you the best price.
To participate in the show, you are agreeing to attend all mandatory rehearsals on the stage, and miss no more than three classes of each subject between January and May. If you miss more than three, the director will make the best decision (regarding participation) that will benefit the entire class. We understand there are exceptions. Check your calendar for vacations, concerts etc. by DECEMBER 1 so you aware of potential conflicts.
The director reserves the right to terminate service to any family as a result of abuse toward any member of the staff or other students. There will be no refund of tuition or costume monies with this termination.
Participation in the annual recital will be terminated to any family as a result of any abuse, or behavior that is negative to the studio. It is the exclusive decision of the director.
Any videos of choreography taken at the studio, stage or any event associated with the dance school, will not appear on facebook, you tube or any other social network without written permission from myself. It is a copyright issue. Please make your child aware of this policy.
In registering my child I understand that dance is a physical activity and an injury may occur.
I will comply with all policies of the school listed above and meet all financial obligations. In the event it becomes necessary
to refer the account for collection, the undersigned parent or legal guardian is liable for all collection fees, and/or attorney fees,
and interest, up to 2% per month, on the balance due.
