COVID-19 Liability Waiver for the Downtown Dance Academy

We have put in place safety and sanitation measures intended to help protect against COVID-19 exposure while at our dance studio/rehearsals/recitals. However, we cannot guarantee that the steps we have taken provide adequate protection or that you (or your family/friends) will not contract COVID-19.

By entering into this studio and/or receiving products and/or services, you accept the risk of exposure and infection to the COVID-19 virus, including, without limitation, personal injury, illness, disability, and death. COVID-19 is a serious illness that is highly contagious. It has spread throughout the world, including our community. Everyone is at risk of getting COVID-19, which can result in severe illness and even death. Older adults and anyone with underlying medical conditions are at greater risk of getting a more severe illness from COVID-19.

I voluntarily agree, therefore, to assume all risks and responsibilities for any such injury, illness, or accident, which might occur to me or my child during any of The Downtown Dance Academy classes, rehearsals, performances, or activities. I understand and accept that my child and I must adhere to the rules set in place to keep dancers/teachers, etc. safe. I also exempt, release, and indemnify The Downtown Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, illness, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by The Downtown Dance Academy. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold The Downtown Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, illness, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. I understand that if anyone in my circle is unwell I should not send my dancer to rehearsals or performances. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of The Downtown Dance Academy classes, rehearsals, performances, or activities. I also exempt, release, and indemnify The Downtown Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by The Downtown Dance Academy. Permission is granted The Downtown Dance Academy to use photographs and videos of students performing for advertising and performance purposes.
