I agree that Inspire Dance Co. is not responsible for any injury or loss which may occur to my child or myself during dance classes, acrobatic arts classes, instructions, performance events or transport to and from any event related to Inspire Dance Co. I have read and understand all rules and regulations of Inspire Dance Co. and agree to pay all fees applicable to my child's 2023/2024 dance year on time (registration, class, costume, festival).

I grant permission to Inspire Dance Company and its agents or employees, to use photographs taken of me or the minor named above for use in studio publications including but not limited to brochures, newsletters, magazines, display boards and to use such photographs in electronic versions of the same publications or on web sites or other electronic forms of media. I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph, and if signing on behalf of the minor named above, I waive all such rights on behalf of the minor named below.
