I release permission to The Arts Alliance and its staff to use my student's likeness and photos taken at the organization's events and classes for promotional, social media, and marketing materials.
The undersigned parent(s) or guardian(s) of the student registering for classes, a minor student at The Arts Alliance, hereby releases and discharges The Arts Alliance, its members and employees, from any and all liability for personal injury suffered by the aforesaid student while on the premises of The Arts Alliance or engaged in activities sponsored or conducted by The Arts Alliances. The undersigned has inspected the premises and is aware of the potential dangers associated with engaging in the activities of various forms of dance and related activities and hereby assumes the risk, on behalf of my or our child or ward, of personal injury. The undersigned further agrees to indemnify and hold harmless The Arts Alliance, its members and employees by or on behalf of my or our child or ward. The Arts Alliance is not responsible for any lost or stolen items left on premises.
I have read and agree to the Fees and Policies outlined in the Parent Handbook by The Arts Alliance. I agree to pay all tuition and/or registration fees by the first of every month or else will incur late fees or reinstatement fees outlined in the policy in the Parent Handbook. I have read and agree to pay all Recital and Costume fees by the due date. The payment agreement plan is made between The Arts Alliance and (Account Holder Name) wherein the (Account Holder) agrees to make payments to The Arts Alliance in accordance with the terms outlined in the terms below: There is a one-time registration fee of $25 for all new students registering for class. All tuition fees are due monthly, based on annual tuition of Aug-May, and due on the 1st of every month. All tuition payments are final and may not be refundable or transferrable. Refunds are at the discretion of the Executive Director based on circumstances or special situations. *Short months are not prorated due to other months being longer. Tuition is not prorated due to family vacations, etc. Late Fee Policy: Tuition is due on the 1st of every month for the upcoming month of classes. A late fee of $15 is applied to each family account for outstanding balances after the 5th of each month. Accounts and student classes are suspended for any unpaid balances after the 15th of each month. Students may not be allowed to participate in class during account suspension. To reinstate an account the outstanding balance, plus any late fees, and re-registration fee of $25 must be paid in full. Any accounts with outstanding balances are sent to collections after 60 days. Class Cancellation Policy: If you wish to cancel your student's class/es, a 30 day notice is required. If less than 30 days is given, you are still responsible for the current/upcoming monthly tuition payment.
