I hereby authorize Prestige Performing Arts to charge my credit card, on a recurring monthly cycle on the first of each month beginning on the date I have registered for classes and enrolled my child at Prestige Performing Arts. I understand that the autopay process will show installments and other charges to be electronically debited to my account on the 25th of each month. If I do not choose another form of payment (Cash, Check or Zelle) my credit card will be charged on the 1st of the month. The last two installments (May & June tuition) will be due May 1st. I understand that a 3.05% processing fee plus ¢.30 cents will be added to all credit card transactions. I understand that Prestige Performing Arts will notify me of any costume balances or additional charges as it is debited to my account during the year. This authorization is to remain in effect until Prestige Performing Arts has received written notification of its termination.

I have read, understand, and agree to comply with the policies as stated on Prestige Performing Arts' portal. I understand that if my child wishes to drop a class, a written notification must be made to Prestige Performing Arts via email prior to the 1st of the month. If an email is not received, I understand that I am still responsible for that month’s tuition, as a place in class is held for my child. I understand that I will be charged a full month’s tuition regardless of which day of the current month I withdraw my child and give Prestige Performing Arts permission to charge that full month to my account. I also understand that registration, tuition, costumes and other payments are nonrefundable. When I choose to order a costume I am agreeing to pay for the costume in full, (both the deposit and the balance) regardless of recital participation. All costumes will be distributed when all account balances are paid.

Please read carefully and sign that you have reviewed and agree to the following policies to secure registration at Prestige Performing Arts (PPA).
Please note that all accounts and fees due can be seen on the parent portal that you have created.

1. I understand that tuition is a yearly price, split up into 10 monthly installments for convenience. This means that the same monthly payment is due regardless of the number of classes in a particular month.
2. I understand that a Registration Fee ($30 per child or $40 per family) and First Month's tuition is due upon enrollment. The last 2 installments of May and June tuition will be due on May 1st.
3. I understand that it is required to have an active credit card on file.
4. I understand tuition will be billed on the 25th of the preceding month and is due by the 1st. To avoid additional credit card processing fees you can pay via Cash, Check or Zelle. If payment is not made BEFORE the 1st of the month, your credit card on file will be charged on that day.

5. If a credit card is declined an automatic email will be sent. I understand if I do not give further instructions the card will be charged again on the 5th of the month. If it is declined for the second time another email will be sent and an additional $20 nonrefundable late fee will be charged to my account. This fee continues monthly until the tuition is paid in full. I understand making payments on time will avoid accumulating any additional charges.
6. I understand that there will be a $35 fee for all bounced checks.
7. I understand that classes are not prorated, and that all fees such as registration, tuition, costumes, jewelry, etc. paid to PPA are NONREFUNDABLE.
8. I understand that PPA instructors reserve the right to dismiss any student from class who is continuously disruptive towards teachers or peers. All students are expected to behave in a respectful manner.
9. I understand PPA has a required dress code to attend class.
10. PPA will communicate upcoming events and deadlines via email. I understand that it is my responsibility to read the emails for important updated information.
11. I understand as a parent/guardian of a PPA student I grant permission for photos/videos of my registered child to be used to promote its studio on their website, Facebook, Instagram, Social Media pages etc.
12. I am aware that my child's cell phone/valuables should be placed safely in a cubby during class and that PPA will not be held accountable for any lost or missing items.
13. I am aware that my child may not use or check their cell phone during class.
14. I am aware that there is a cancellation policy in place for all private lessons. PPA requires 24 hours notice for a private lesson cancellation. (Please keep in mind that not only is space set aside for the individual, but the instructor has set aside time and travel for that lesson). If less than 24 hour cancellation notice is given PPA will be compensated for the cost of the lesson in its entirety.
15. I understand that there will be a mandatory $30 Recital Video Fee billed to each family's account on April 1st. This guarantees a digital download for all Recital shows for each family. This fee only applies to Recital & Competition Families.
16. I UNDERSTAND CLASSES ARE NOT PRORATED. IF A CHILD WILL NOT BE ABLE TO ATTEND THE FULL MONTH, HE/SHE CAN ATTEND A MAKEUP CLASS OF THEIR CHOICE. ALL MAKEUPS MUST BE DONE WITHIN THE CURRENT SEASON.
17. I UNDERSTAND IF A CHILD WITHDRAWS FROM A CLASS A WRITTEN NOTICE VIA EMAIL PRIOR TO THE NEXT BILLING CYCLE IS REQUIRED. IF A CLASS IS DROPPED ON THE FIRST OF THE MONTH OR AFTER I WILL BE RESPONSIBLE FOR PAYING THAT CURRENT MONTH.

I assume all financial responsibility for the above student enrolled at Prestige Performing Arts. I have read and will follow all rules and policies of PPA.

I do hereby release Prestige Performing Arts, and all other instructors and/or students, in any capacity for all liability due to injuries, etc. that I/my child may obtain as a result of attendance or participation in any dance/acting/theater class, other related activities or any special event at Prestige Performing Arts. I clearly understand that participation in any of these classes will involve bodily exercise and movements with vigorous activity. I am fully aware of my/my child's personal condition and hereby certify that I/my child are mentally and physically fit to participate in the classes and that I have medical coverage or personal means to cover the expenses related to any injury that I/my child may receive as a result of participation.
