I understand that there are risks of physical injury associated with, arising out of and inherent to
the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily
waive all right and/ or causes of action of any kind, including any and all claims of negligence
arising as a result of such activity from which liability could accrue to AATMA Group LLC, its
officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated
entities (hereinafter collectively referred to as “AATMA Performing Arts”.
I hereby agree to release AATMA Group LLC and AATMA Performing Arts harmless of all
liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all
risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing
assumption of the risk of injury. I have signed this document voluntarily and of my own free will in
exchange for the privilege of participation.

I also give AATMA Performing Arts permission to use my child’s picture in or on any form of
advertisement for AATMA Performing Arts or an AATMA Performing Arts affiliated event.
If I am a minor, my parent and/or legal guardian has also signed this document releasing AATMA Performing Arts from any and all such liability described above and has acknowledged that I am
knowingly and voluntarily assuming all risks of injury inherent to this activity.
The participant has my permission to participate in AATMA Performing Arts events and shows. I warrant the
below information is complete and correct. I further release AATMA Group LLC of all liabilities
associated with my child’s attendance at AATMA Performing Arts.

I have read/understand AATMA's school policies & tuition rates and agree to the terms as outlined below:

Annual Commitment
AATMA tuition rates for dance classes are based on a full year (12-month) commitment from the date of enrollment. Besides Bollywood Cardio, all students are enrolled in a 12-month curriculum with the option to take a 1-month pause between July-January.

Down Payment/Membership Fee: $199
This payment is due at the time of enrollment and is part of the annual tuition rate. This fee is charged every 12 months to maintain your membership at AATMA.

Payment Plans (Monthly)
By enrolling in our classes, you agree to have payments automatically taken on the 1st of each month (via Auto-Pay) via credit card or bank account. You also agree that there are no refunds or cancellations after payment is processed.

Flexibility
We know every family needs a break from time to time. Although students are expected to be committed for the entire year, you can be granted a 1-month pause on your tuition with 60 days' written notice. (For example, if you would like to pause your enrollment for the month of July, you must fill out a membership pause form and submit it no later than April 30). Please note: We will not allow any pauses between the months of January-June due to recital preparation.

Membership Cancellations
We understand that unexpected situations may arise, and you may need to withdraw from our program. If you wish to cancel your AATMA membership, you must provide us with written notice before your next charge and fill out a membership withdrawal form. There is a 3-month penalty for breaking the annual commitment. (For example, if you wish to discontinue your enrollment in March, you will be charged for the months of April, May, and June).
