I have throughly read all of Life's Your Stage Dance Company's Policies and Procedures including the payment and privacy policies and agree to adhere to each outlined policy.

Life’s Your Stage Dance Company Required Health Release:

I hereby release my dancer to participate in a high level of physical activity at Life’s Your Stage Dance Company. I understand that a certain level of risk of physical injury naturally accompanies participating in such activity and I will not hold Life’s Your Stage Dance Company liable for any injury or other health complication that occurs either at the studio location or because of any other activity involved with the studio. This includes any illness that may be contracted by attending in person classes. I understand that Life’s Your Stage will not administer any kind of medical treatment under any circumstances and is not responsible for any complications caused by self-medication or treatment at the studio location. I understand that this document is legal and binding and that my acceptance below indicates my full understanding and adherence to this statement.

I hereby release Life's Your Stage Dance Company and all associated parties of any liability of injury that may be incurred by participating in their online classes, whether they be live streamed or pre-recorded. I understand that any physical activity involves some risk of injury and that it is my responsibility to ensure that these classes are performed in a safe, open space in my home.

I have throughly read Life's Your Stage's Policies and Procedures including the Payment policy and the Privacy policy and agree to adhere to all of the described policies. I fully understand that failure to adhere to these policies may result in disciplinary action including but not limited to monetary fines or termination of my contract. Life's Your Stage reserves the right to adjust any of these policies with a full month's notice given before a change goes into effect.

I hereby authorize Life’s Your Stage Dance Company and its employees to photograph and/or video tape my child while involved with Life’s Your Stage activities and use those materials for promotional purposes.
