SAFE DANCE PRACTICE POLICY
I understand that my child will undertake dance/cheer training and whilst all care is taken and safe teaching practice is observed, accidents can occur. I agree that first aid measures will be applied in the case of an accident or emergency and if necessary, should I not be contactable, my child will be transported to hospital via ambulance. I also agree to notify SDCA if any illness or injuries are sustained to my child outside of classes so that all care can be taken.
I acknowledge that my child's class may be filmed or photographed from time to time in order to study technique and as a teaching aid. Photographs and footage may also be used for promotional purposes.
Fees are payable per month via our secure payment portal. You may also receive invoices for costume fees or competition entry fees depending on which class you are enrolled in.
Your first payment will also include your enrolment fee of $20 per child (Capped at $40) which covers yearly student insurance and administration.
Should the services of a debt collection agency be required to collect an outstanding debt on an account then the parent listed on the account will be liable for all resulting additional fees as billed by the agency. Note, any discounts that may have been applied will be forfeited. All fees are non refundable and non transferable.
If you wish to discontinue classes you must notify SDCA via email and you will be charged until the end of the current monthly payment cycle.
I authorise the above information to be correct. I have read and agreed to all of the terms stated in the safe dance practice, media and payment policies. I am also aware that SDCA have student and parent codes of conduct that protect the safety and enjoyment of SDCA families and staff.
