Release and Liability Waiver I hereby authorize Moonstar Dance Studio and all of its employees to instruct my child in the art of dance and related activities. I recognize the inherent dangers associated with such activities. I understand that while Moonstar Dance Studio will take reasonable care to instruct, there are numerous injuries which may occur in the course of that instruction. I hereby release and absolve Moonstar Dance Studio and its employees from all claims and causes of action arising out of their care and instruction.
