Ticket sales help pay for performances. Please read your class description carefully to see where your show will be and how much ticket sales will be. Some classes may not have end-of-the-session shows. Tuition prices are listed in class description and are due on the first of each month. Performance fees are due upon enrollment and are non refundable. All final balance payments must be submitted by the first day of the fourth month of the session. $35 returned check or card fee. All payments are non-refundable. Discounts: sibling discount is $5 per month for each sibling. Performance dates are usually listed in each class description. Theatre Show Tickets could be as low as $5 or as high as $15 depending on venue. Performance dates are TBA. Pro-Rating: You do not owe more tuition for months that have more weeks. You do not owe less tuition for months with less weeks. If you are absent for an entire month, that month's tuition is still due. Students who join late will still need to be taught the material they missed earlier in the session. Tuition will not be pro-rated. The full session's tuition is still due. For safety reasons, please do not drop off your child early for class. Please be prompt in picking them up right when their class ends. We do not have extra staff members to watch students whose parents are not there with them. Only enrolled students and teachers are allowed in the classroom. Additional people cause distractions. Injury Recognition Release I understand that dance, cheer, tumbling, gymnastics, and movement activities are physical activities which can potentially lend themselves to serious injury. I am willing to assume these risks for my child/children and/or myself. I hereby release Main Street Dance Works, LLC, its employees, owners, volunteers, their families and their heirs from any liability due to any injury (injuries) sustained or illness contracted by my child/children and/or myself while participating in any Main Street Dance Works activity. It is each family's responsibility to supply medical insurance for all participants. • By completing the information below, you are stating that you have read, understand and agree to all of the information listed on this enrollment form. • All parents/guardians should be made aware of the information. • Parents please inform us in writing within 5 days of enrolling if you do not want us to use your child's pictures for publicity purposes.
