PARTICIPATION LIABILITY WAIVER
I/We __________________________, the parent(s)/legal guardian of __________________________ understand that classes will involve physical activity and risk of injury. I assume the risk and upon reading and accepting this agreement do hereby release, discharge and hold harmless and safe The Dance Conservatory of Pittsburgh, Inc., its directors, employees, instructors, chaperones, agents and officers from any and all present and future, liability, civil or criminal claims, demand, rights, actions, damages, expenses, compensation and all known and unknown personal injuries, death, loss of property and/or property damage during attendance at The Dance Conservatory of Pittsburgh or any of its related functions. I also give permission for said minor to be treated for medical emergencies. The Dance Conservatory of Pittsburgh reserves the right to terminate and/or refuse service at any point.
COVID-19 LIABILITY WAIVER and ASSUMPTION OF RISK
I/We __________________________, the parent(s)/legal guardian of __________________________ understand that The Dance Conservatory of Pittsburgh will comply with all CDC, state, and federal mandates and/or guidelines regarding mitigation of COVID-19. I shall indemnify, defend and hold harmless The Dance Conservatory of Pittsburgh, Inc., its directors, employees, instructors, chaperones, agents and officers from and against any and all claims, demands, suits, judgments, losses or expenses of any nature whatsoever arising from or out of, or relating to, directly or indirectly, the infection of COVID-19 or any other illness. I knowingly assume all risks, known and unknown, and assume full responsibility for the participation of myself and/or said minor. Additionally, I agree to be forthright and true with The Dance Conservatory of Pittsburgh should I or said minor participant show symptoms of illness, test positive for COVID-19, come in contact with people or persons showing symptoms of illness, or come in contact with people or persons who have tested positive for COVID-19.
ENROLLMENT OPTIONS
1. Enrollment in the complete 2024-2025 session with payment in full.
2. Month to Month: Enrollment for the 2024-2025 session with agreement to pay tuition as a recurring dues membership that automatically renews on a month-to-month basis. Cancellation of month-to-month enrollment is permitted with 30 days advance notice.
PAYMENT OPTIONS
1. Payment in full with complete course enrollment. A 10% discount is extended to payments made for the course in its entirety.
2. Auto Pay. Tuition will be automatically debited from your designated account on the 1st of each month.
In the case of insufficient funds, you will be notified and the account will be charged again with the addition of a $30 fee.
3. Monthly Payments. Monthly payments will only be accepted in the form of cash. Payments are due by the 1st of each month.
A $20 late fee will be strictly applied to any payments received after the 1st. A 12% finance charge will be applied to all balances 90 days past due.
2024 – 2025 POLICIES
-There is a nonrefundable registration fee of $30 for an individual or $35 for a family.
-Tuition is calculated at a per month rate. All tuition is non-refundable.
-There is a 10% second student discount and a 15% discount for families with 3 or more students.
-Monthly tuition payments are debited from specified accounts on the 1st of the month.
-For accounts not participating in auto pay, tuition payments are due by the 1st of each month, in cash.
-For payments received after the 1st a $20 fee will be charged to your account.
-Any bounced check or insufficient funds on a transaction will result in a $30 fee.
-After two bounced checks, DCP will only accept cash as a method of payment.
-There are no refunds for missed lessons. Classes may be made up within one month of the absence.
-Until provided with proper notice of schedule changes or cancellation at least 30 days prior to my desired final month of enrollment recurring tuition and membership with The Dance Conservatory of Pittsburgh will automatically continue, including payment of all dues, fees and charges associated with my account.
-DCP reserves the right to use any student’s picture without name or compensation for its use in advertising and promotional materials. Parent/guardian has the right to opt out of the photo and social media release in the parent portal.
-DCP reserves the right to shorten class duration and deem private instruction for sessions with 3 or less participants in attendance.
-Please call DCP two hours prior to your class time for any cancellations due to inclement weather. In most cases a text and/or email will be sent with class cancellations as well as a post on wpxi. Classes are rarely cancelled.
-Students are to be picked up inside the facility after their class ends promptly. Minors that exit the Cool Springs premises with or without parental permission are not the responsibility of The Dance Conservatory of Pittsburgh.
-Most communication about studio happenings including account statements occur through email. Please provide an email address with registration.
-Participation in the Spring Performance is optional. However, any student that withdraws from the performance after committing will be charged a $50 fee and will still be responsible for any associated costs including choreographer fees and tights purchases. This excludes injury and bereavement.
-Participants must agree to be forthright and true with The Dance Conservatory of Pittsburgh should they show symptoms of illness, test positive for COVID-19, come in contact with people or persons showing symptoms of illness, or come in contact with people or persons who have tested positive for COVID-19.
DRESS CODE
Proper dance attire differs from genre to genre but adherence to dress code is expected at all times. Dress codes are in place to minimize time spent adjusting clothing, and to allow instructors to see the line of the body in order to teach proper technique and prevent injury.
Pre-school: Any Leotard and tights or T-shirt/tank with lose fitting pants or leggings. Ballet shoes and/or black tie tap shoes
Level 1 and higher:
Ballet – Black leotard, skin tone or pink tights, skin tone or pink ballet shoes, hair in a bun
or any form fitting white tank or dance t-shirt and black pants or leggings, hair secured away from face.
Modern, Jazz & Lyrical – Black leotard, black, pink or skin-tone tights with hair securely pulled away from face.
or comfortable dance attire, preferably black pants/leggings/tights and white tank/T-shirt. No shorts.
Hip-Hop - Comfortable clothing. No skirts, boots, flip flops, or elevated shoes. Sneakers preferred. Baggy clothing acceptable.
Tap – Comfortable dance attire. No pants that drag on the floor/no jeans. Feet must be visible. Hair securely pulled away from face.
*Students may wear a colored leotard to all classes during the first week of each month
At DCP we believe all students should feel welcome, safe
and included in their daily lives as their authentic selves.
Our dress code is gender-neutral, and every dancer
should wear which option feels appropriate to them.
