Creative Motion School of Dance
Policy & Waiver
*Over the course of the dance year, photos and video may be taken of my student. I hereby give permission for images of my child, captured during regular and special dance events throughout the year to be used solely for the purpose of promotional materials and publications, and hereby waive any rights of compensation or ownership thereto.
*Monthly tuition payments are due by the beginning of the month. Any account falling past due will be subject to a $10 or 2.5% late fee, which ever is greater, per month overdue and will forfeit any applicable discounts. There will be a $25 fee charged for any returned checks.
*Dance class begins and ends on time according to the studio clocks (set to satellite time). I will make it a priority to have my child at the studio 5-10 minutes (but no more) before their scheduled class time and will pick them up promptly when their class is dismissed.
*Creative Motion expects that each student will come to class ready to learn. I understand that Creative Motion expects that students will respect themselves, their fellow students and their instructors. Disrespect will not be tolerated.
*Creative Motion dancers are expected to abide by the studio dress code. The dress code helps instructors correct form and technique and gives the student the freedom to move appropriately and safely in class.
*There will be designated observation periods over the course of the year. Creative Motion has observed through its years of experience that the less distractions in the studio, the quicker the students progress!
*I understand the studio inclement weather policy, and know when in doubt to check the studio website and/or the Facebook page. Class decisions are generally made by 1:00pm.
*If I should need to withdraw from class that the registration fee and costume fee are non-refundable. I also understand that tuition will be charged/refunded on a prorated schedule - monthly through one month following receipt of the formal withdrawal. Failure to attend class does not mean withdrawal. The withdrawal form must be requested, filled out and returned to the studio.
*Members of the Creative Motion staff have all students' best interests in mind when placing students in classes. Sometimes students need to be placed by technique instead of age. I agree to allow Creative Motion to place my child as they see most beneficial.
*Creative Motion offers business hours and I will try my best to address any questions, comments or concerns during those business hours instead of before or after my child's class. Creative Motion is able to respond quickest via email, but also know that I can call or make an appointment to stop in as well.
*Creative Motion has an open door policy and encourages students or parents to ask questions or address any concerns they have. I understand that many times situations that lead to frustrations usually stem from misunderstanding or misinformation. I will make an effort to speak to a member of the Creative Motion Staff before expressing my concerns to other parents to be given an opportunity to receive all the information.
*I understand that it is my responsibility to stay informed and will utilize the family handbook, website, emails and Facebook page.
*I fully understand that Creative Motion staff members are not physicians or medical practitioners of any kind. With that knowledge, I hereby release the Creative Motion staff to render temporary first aid to my child or children in the event of any injury or illness, and if deemed necessary by the Creative Motion staff, to call a doctor and seek medical help, including transportation by Creative Motion staff member or representatives, whether paid or volunteer, to any health care facility or hospital, or calling an ambulance for said child should the Creative Motion Staff deem this to be necessary.
Creative Motion and its instructors will not accept responsibility for injuries sustained by any student during the course of class, rehearsal or performance of dance or while traveling to or from an organized event.
*I understand the above and I am aware of the risks and the possibility for injury involved. I consent to have my child or children participate in the programs offered by Creative Motion. I, my executors or other representatives, waive and release all right and claims for damages that I or my child may have against Creative Motion and its representatives whether paid or volunteer.
*I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage that I consider adequate for both my child's protection and my own protection. I have also informed the studio, in writing, of any physical or mental special needs my child may have (including allergies). This written notice will be submitted no later than the first class meeting.
