By registering for classes/camps, you are agreeing to the following:
*I give permission for any and all medical attention to be administered to my child(ren), in the event of accident, injury, sickness, etc., under the direction of the instructor, Studio owner, until such time as I may be contacted. I also assume the responsibility for the payment of any such treatment.

*I give full rights to the Fuzion Dance Studio to use photos and/or video images of me or my child to use for promotional purposes of the Fuzion Dance Studio only. Photos and videos may be used in brochures, websites, advertisements, and other promotional material created by the Studio. Photos may appear with or without names in press releases and other print advertising.

*I have read, understand and agree to the above stated waiver of liability, medical and photo releases. I have also read and understand the “Fuzion Dance Studio Handbook”. I understand I will be held financially responsible for all tuition, costume payments, and late fees, etc. as set forth in the handbook and agree to comply with the Studio policies and rules.

I waive the right to any legal action against Fuzion Dance Studio, the owner, or staff and 514 Archer Ave.(Marshall) and/or 120 N. Main (Paris), the owner, or staff, for any injuries sustained on the property or at any Fuzion Dance Studio event. I understand that I am enrolling my child or myself in a program of physical activity and have agreed that my student/myself is in good physical condition and does not suffer from any condition or disability that would prevent or limit participation in this program. I further understand the potential risk and that I must provide my own insurance.

I understand the Fuzion Dance Studio has put in place preventative measures to reduce the spread of COVID-19 and the seasonal colds/flu/illnesses; however, the Fuzion Dance Studio cannot guarantee that you or your child(ren) will not be exposed. By registering for classes, you acknowledge that you have made an informed and conscious decision to participate in in-Studio classes; you acknowledge the contagious nature of COVID-19 and the seasonal colds/flu/illnesses; and voluntarily assume the risk that you and/or your child(ren) may be exposed to or infected. You understand that the risk of becoming exposed to or infected may result from actions, omissions, or negligence of yourself or others. Finally, you agree to indemnify and hold harmless the Fuzion Dance Studio, its staff, and participants for any infection you may get infected with while participating in this program.
