LIABILITY RELEASE INFO I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. I waive the right to any legal action against Dance With Me Studio and/or KB Studios, LLC for any injury sustained on studio property or at any Dance With Me Studio event. I understand that I am enrolling my child in a program of physical activity and have agreed that my student is in good physical condition and does not suffer from any disability that would prevent or limit participation in this dance program. PHOTO & VIDEO RELEASE INFO I give full rights to Dance With Me Studio and its staff to use photos and video images of me or my child(ren) to use for promotional purposes of Dance With Me Studio only. Photos and video may be used in brochures, websites, advertisements, and other promotional material created by Dance With Me Studio. Photos may appear with or without names in press releases and other print advertising. STUDIO HANDBOOK AND CORRESPONDANCE I have received the student handbook and agree to adhere to all the content stated therein including but not limited to: Studio Policies Class Fees & Payment Information Dress Code Traffic Pattern Visitor Weeks Calendar I agree to be responsible for reading studio correspondence and respecting deadlines when applicable. I hereby acknowledge that I have read, understand and agree to the statements above and agree to participate accordingly.
