Dance, Tumbling, Aerial Hoops, and Aerial Hammocks Participation Waiver
I hereby acknowledge and agree to the terms and conditions of participation in dance, tumbling, aerial hoops, and aerial hammocks activities conducted by Valley Pointe Dance Co. In consideration of being allowed to participate in these activities, I willingly agree to the following:
Acknowledgment of Risks: I understand that participating in dance, tumbling, aerial hoops, and aerial hammocks activities involves inherent risks and that injuries can occur. These injuries may range from minor to severe, including but not limited to sprains, fractures, or more serious injuries.
Health and Fitness: I certify that I am in good health and physical condition and that I have no medical conditions that could prevent my safe participation in these activities. I will inform the instructors and staff of any changes in my health or medical condition that may affect my ability to participate safely.
Assumption of Risk: I acknowledge that I am voluntarily participating in these activities with full knowledge of the risks involved. I assume all responsibility for any injury, damage, or loss that I may sustain as a result of my participation.
Release of Liability: I release Valley Pointe Dance, its owners, employees, instructors, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injuries, damages, or losses that I may incur while participating in these activities.
Photo and Video Consent: I grant Valley Pointe Dance the right to use photographs and videos taken during the activities for promotional and marketing purposes. If I do not wish to be photographed or filmed, I will inform the instructors or staff in advance.
Emergency Medical Treatment: In the event of an injury or medical emergency, I authorize Valley Pointe Dance to seek appropriate medical treatment on my behalf. I understand that I will be responsible for any associated medical expenses.
Compliance with Rules: I agree to comply with all safety guidelines, rules, and instructions provided by the instructors and staff. Failure to do so may result in my removal from the activities.
Code of Conduct: I will conduct myself in a respectful and appropriate manner towards all instructors, staff, and fellow participants, maintaining a positive and inclusive atmosphere.
I have read and understood this Participation Waiver, and I sign it voluntarily, fully aware of its legal consequences. I understand that by signing this document, I am giving up certain legal rights, and I do so willingly and without any coercion.
