In consideration of my child's participation in the classes given by RISE Performing Arts Center, I do herby for my child release and discharge Brecia Provost and all personnel thereof for all claims or damages, demands, actions, injuries, or whatsoever in any manner arising or growing out of my child's participation in any of the classes offered by RISE Performing Arts Center. I attest and verify that I have full knowledge of the risks involved in exercise and my child is physically fit and able to participate without endangering my child's health.

First month tuition and registration fee are due on or before the first day of students first class. There is a $40 registration fee per student, any additional family members are $10 per family member. (This does not apply to Summer classes.)

Tuition is due on the 1st day of each month. A late fee of $10 will be added to any payment after the 5th of the month. Additionally, a $35 fee will be charged for any returned checks. We do not prorate. We offer a 10% discount on sibling tuition. We also offer a discounted rate if tuition and fees are paid in full for the whole year.

Competition team fees and costume fees will be handled separately.

There will be a $50 recital fee per student due April 1st. Recital date is TBD.

Payment is due in full on first day of class. Payments can be made on or before first day of classes. Cash and check are accepted. There is a $25 return check fee. Credit cards are also accepted. There is a 5% charge for credit card use.

Students with accounts more than 2 months past due are subject to being dropped from classes. Similarly, accounts must be current in order to receive costumes and participate in recital.
