By signing below I acknowledge that I have read and understand these Song and Dance policies: - Song and Dance, Inc. is NOT LIABLE for personal injury. - There is a $40.00 charge for all returned checks. - Upon registration, I am responsible for the monthly tuition. - Tuition is due by the 15th of each month. COVID-19 Warning We have taken enhanced health and safety measures—for you, our Dancers, and Observers. You must follow all posted instructions while visiting Song and Dance Inc. An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and Guests with underlying medical conditions are especially vulnerable. By visiting or dancing with Song and Dance Inc. you voluntarily assume all risks related to exposure to COVID-19. Help keep each other healthy.
MEDIA RELEASE I hereby give permission to Song and Dance Inc. to use, re-use, publish, and re-publish any photos or videos of my child or containing my child for promotional purposes. This includes but is not limited to: illustration, promotion, display, advertisement, and trade. I hereby release and discharge Song and Dance Inc. from all and any claims and demands ensuing from or in connection with the use of the photos and or videos, including any and all claims for liable or invasion of privacy.
