I/We accept and assume all risks and responsibility for the losses and/or damages following any injury or disability, however caused or alleged to be caused in whole or in part by the negligence of The Mid-Shore Dance Academy, other participants, instructors, sponsors, directors, event hosts, agents, and employees.

Class Times
Please do not sign up for classes that you cannot attend fully. It is disruptive to the entire class if students arrive late and/or leave early and students miss warm-up at the beginning and skill development at the end. We understand the occasional need to make an adjustment for a special event. Please just let the instructor know ahead of time and enter/exit as quietly as possible.
Please arrive BEFORE your scheduled class time to be READY on time!

Drop-off and Pick-up
STUDENTS UNDER THE AGE OF NINE MUST BE ACCOMPANIED BY A PARENT OR SIBLING AGED 12 OR OLDER INTO/OUT OF THE BUILDING FOR DROP-OFF AND PICK-UP!
This is for your child's safety, and is based on state law that children under the age of nine cannot be left unaccompanied without supervision. It is also the parent(s)' responsibility to make sure your child is prepared for class before you leave (hair up, shoes on, wearing proper attire).

Please be prompt in picking up students. Young students may get distressed if a parent is not present and teachers cannot monitor children left from a previous class when they start teaching another class.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend physical distancing and have, in many locations, prohibited the congregation of groups of people. The Mid Shore Dance Academy has created new protocols and put in place preventative measures to reduce the spread of COVID-19. However, MSDA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, in spite of the measures in place, attending any program may increase your child(ren)s risk of contracting COVID-19.

By completing registration for classes, I am agreeing to the following:

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to, or infected by COVID-19 by attending any Mid Shore Dance Academy (MSDA) Programs, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the MSDA program may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MSDA employees, volunteers, and program participants and their families. I understand that any or all programs may be cancelled, without warning, if a suspected or positive case of COVID-19 is presented. I understand that if my child(ren) or myself voluntarily omits following MSDA COVID-19 protocols, my child(ren) will not be allowed to participate and will be removed from the program. I voluntarily agree to assume all foregoing risks and accept sole responsibility for any injury to my child(ren) or myself including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)s attendance at the MSDA program. On my behalf, and on behalf of my child(ren)s, I hereby release, covenant not to sue, discharge, and hold harmless The Mid Shore Dance Academy, LLC and its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MSDA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any MSDA program.

Accepted forms of payment: cash, check*, credit/debit card (Visa, Mastercard, or Discover), ACH
*A $40 fee will be assessed to your account for checks returned by the bank.
*Post-dated checks are not accepted.

Fee Details / Procedures:
1. At the time of registration, a non-refundable 20% deposit is paid. This reserves your space in class(es).
2. The online registration system will automatically calculate multi-class and sibling enrollment discounts.
3. The balance of the session fee will be automatically billed during the week prior to the start of the session, unless the studio has been notified of cancellation.
4. All accounts are required to register for "Auto-Pay" and have a valid payment method on file, so session balances can be processed.
5. Tuition is not discounted for classes missed due to illness, vacation, or other absences. Students can attend another class for make-up.
6. Tuition payments are non-refundable, except in the event of class cancellation by the studio.
7. Enrolled students will be automatically re-enrolled into subsequent sessions, and fees processed accordingly, unless the studio is notified by the discontinue date as listed on our calendar. This does not apply to Toddler Sessions. Re-enrollment required for these.
8. Note that Monday fall session class fees have been reduced due to Labor Day. Subsequent session fees will be at the regular rate.
8. Monthly Installment Option: For students/families in 3 or more classes, monthly installment payments can be arranged instead of session fees. Full School-Year commitment required. At time of enrollment, the 20% 1st session enrollment deposit is collected automatically. Contact the studio to set up monthly installments, and remaining 80% of 1st session, plus remaining sessions will be divided into 9 monthly payments, Oct-June.
