Injury/Illness Liability Release
I am aware that dance training and the athletic exercises associated with it will place unusual stress on the body and carry the risk of physical injury. On behalf of my child and myself, (and if I am no longer a minor on my own behalf), I assume the risk and agree that Dance Ascension, LLC (DA) shall not be liable in any way for injuries sustained during attendance at DA or any of its related functions and during transportation to and from functions.
As a condition of use of Dance Ascension, LLC for participation of any class, camp, intensive, or activity, you agree as a participant or parent/guardian of a participant in the program, to assume all risks and hazards of engaging in or spectating at Dance Ascension, LLC or any other activity or facility, including assuming all risks for personal injury, death, and property damage.
You agree to hold harmless Dance Ascension, LLC and its instructors, volunteers, guest instructors, or owner from liability for any losses, damages, or injuries that may occur in any way as a result of these activities or facilities- regardless of cause.
Participants (or the parents/guardians of minor children) may also be required to complete a medical health history and/or sign an additional waiver of liability prior to participation in any programs or classes. Anyone recovering from an injury or illness must first discuss their return to physical activity with their physician and Dance Ascension, LLC prior to resuming classes or other programs offered by Dance Ascension, LLC.
You further agree that there is risk associated to the COVID-19 pandemic and possible exposure.
Therefore, having read and understood this waiver and in consideration of your accepting of my entry into Dance Ascension, LLC or other facilities Dance Ascension, LLC may use or attend as part of recitals, performances, competitions, conventions, etc, I, for myself and anyone entitled to act on my behalf, waive and release Dance Ascension, LLC, its instructors, volunteers, guest instructors, and owner from all claims or liabilities of any kind arising out of my participation in (or as spectator of) any program or use of any facility.

I hereby authorize DA to record the student’s picture and voice on photographs, films, and tapes. I give my permission for DA to use these materials for publicity, advertising and sales. I acknowledge that no promises of compensation were made by DA for such use.

In the event I cannot be reached, I hereby give my permission to the management, faculty, staff and chaperones of DA to authorize any emergency medical care that may be required by the above student during participation in classes, performances, or any related DA events. This authorization extends throughout the current academic year and throughout the summer or until the student is no longer enrolled at DA, whichever comes first. I understand that I am responsible for any and all charges as a result of such care or medical treatment.

You agree and understand any and all fees for tuition, registration fees, camp/workshop fees, costumes fees, recital fees, etc are NON REFUNDABLE.

Deposits for camps/workshops are NON REFUNDABLE. No exceptions

I agree to pay DA for the class instruction of the student listed above per the published tuition rates for the student’s period of study. I understand that I can make payment by automatic draft, bill pay through my bank, or Venmo. I understand that DA is not accepting checks or cash due to COVID-19. I understand that NO REFUNDS, prorated rates, or discounts will be given for classes missed because of illness, vacation, studio closings due to acts of nature such as inclement weather or government mandated shutdown. I understand that tuition fees are due on the first of each month; that a late fee will be charged on the fifth of each month for tuition not paid of $10 for each month that payment is late. If tuition is late three (3) times within one dance season, August through July, the student will be dismissed from the studio. In the event a student must be dismissed due to late tuition payments, said student will not be allowed to register for classes, privates, camps, or workshops for a full 12 months.

You further agree and understand any and all fees for tuition, registration fees, camp/workshop fees, costumes fees, recital fees, etc are NON REFUNDABLE. No exceptions. Summer camps/workshops must be paid in full prior to the first day of the camp or the student will be dropped from the camp. If you decide to withdrawal from a dance class (not a summer camp/workshop), please notify the office via email (danceascension.info@gmail.com) to withdraw yourself or your student. No further charges will accrue once withdrawn notice has been given. While we do not offer refunds for fees or tuition, we do offer make up classes in the event classes are missed due to illnesses or to what we consider an excessive amount of classes missed due to inclement weather. A student may take any equivalent class as a make up class. Please notify the office if you plan to attend a different class to make up missed classes.
