I understand that there are no refunds for fees or tuition. I agree that I will not hold Arkansas Dance Center, its faculty, staff or volunteers for any injuries that may occur on or outside the premises.

I understand that by accepting this, I agree to not hold Arkansas Dance Center, its teachers, guest instructors, facilities, or staff liable for any accident or injuries that may occur while participating in activities on or off the premises.

I understand that payments made to Arkansas Dance Center are non refundable.

I also understand that when participating in auto pay, I must give written notice of cancellation at least five(5) business days prior to the effective date of cancellation.
