PARENT/GUARDIAN ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY / MEDIA AND TUITION POLICIES

Liability Waiver and Acknowledgment of Risk:
I understand and agree that in participating in any dance and tumbling class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I recognize the potential for physical injury, even serious or catastrophic injury, that could occur in any activity involving height or motion including dance, gymnastics, and related activities including tumbling and Arial rotations. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Jete Dance Arts & Athletics classes, rehearsals, performances, or activities. I also exempt, release, and indemnify , its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Jete Dance Arts & Athletics. Furthermore, any medical expenses, if an injury should be sustained by my child while training or performing during a Jeté Dance Arts Activity, will be my individual sole responsibility as legal guardian of the student. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Jeté Dance Arts & Athletics, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. By submitting this form for registration and enrollment for the 2024-2025 Season at Jeté Dance Arts & Athletics I am signing this waiver and policy for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I have read, understood and agree to be bound by the above statements and policies of Jete Dance Arts & Athletics

I agree to the following tuition/payment policy. Tuition is based on enrollment not attendance. Tuition rates are $60 first hour ; 25% discount 2nd hour and 50% discount for each hour thereafter. 15% sibling discount. Missed Classes will not be refunded for any reason including illness injury other sports commitments or holidays. Jeté Studio follows the Huntsville School District Calendar for Holidays and Jeté Studio will be closed when school is closed for holidays. Studio cancellations due to weather will be rescheduled as is possible. Tuition is calculated by the entire number of classes in the season and divided into nine equal monthly payments from September to May. Fall classes 2024 will begin the week of August 19th and August tuition will be 1/2 the month tuition. Registration and recital
Fees will be due upon enrollment and are not refundable. You will not be refunded for months with fewer than four weeks and you will not be charged for months with more than four weeks. After Initial Registration and First Months Paid Tuition All Recurring Monthly Tuition is due the first day of class each month unless special arrangements are made . Tuition is late after the first week of class. IF TUITION for my student(s) is paid after the 8th of the month I agree to add a $15.00 late fee to my tuition payment. Please note, Jeté Dance Arts does not have a pay by class program, and it is the responsibility of the student to make up any missed classes. Tuition will not be discounted for missed classes, holidays or snow days.

Media Release:
I give permission for my child/children to be photographed and/or videotaped, without compensation, and to have those pictures appear in any paper or electronic means or media, for the sole purpose of promotion and advertising of the Jeté Dance Arts & Athletics studio.
If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I have read, understood and agree to be bound by the above statement.
