FLOURISH WAIVER 2023-2024

I recognize that the program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. I hereby affirm that the child I am signing for is in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this program . I affirm that I alone am responsible to decide whether to allow this child practices and participates. I will indemnify and hold harmless Flourish Belton, LLC. and its employees and volunteers from any and all claims, losses or expenses arising out of the performance of this contract including all litigation expenses and reasonably attorney’s fees. I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
