ACH AGREEMENT
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (once online payment is made or autopay information is provided) this facility to charge my ACH draft, or debit/credit card account. I understand that all payments will be collected 30 days in advance. I have read and understand Inspire School of Ballet drop procedures and acknowledge that I am responsible for payment, whether or not my child attends classes, until I submit my drop request (available online via the parent portal) using the studio's official Withdrawal Form 30 days in advance. Inspire School of Ballet does not provide refunds, no exceptions.
If I dispute a charge through my financial institution, this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services. If my bank account is compromised, I will immediately advise Inspire School of Ballet, and will provide alternate payment to remain current and/or replace any unpaid ACH charges and any fees charged by ISB’s bank.
ANNUAL MEMBERSHIP FEE
Classes begin July 15, 2024 and end June 15, 2025. All Students will be charged an annual membership fee of $65 or a family membership fee of $75 upon enrollment and our Monthly tuition based on how many classes they enroll in.
ENROLLMENT AND TUITION
Classes are provided as a continuous enrollment, meaning you can join at anytime! All tuition is billed 30 days in advance, on or about the 1st day of each month. At sign up you will be charged an annual fee and a tuition amount to cover the current month (prorated if applicable). Electronic billing information is required to be enrolled in class.
BILLING AND PAYMENT
All tuition is charged on the 1st day of each month, and payments are collected by charging the provided billing information. Authorized parent/guardian is responsible for ensuring payment information is accurate and up-to date. ISB reserves the right to drop enrollments for any declined payments and outstanding balances. These payments remain the same regardless of the number of weeks in a month or holidays. Tuition is non-refundable and not-transferable.
Late fees: If your tuition is declined, you have until the 5th of that month to pay your tuition before incurring a $35 late fee. Declined checks are subject to a $35 fee.
Inspire School of Ballet reserves the right to conduct classes virtually in the event that classes are not able to be held in person for any reason including but not limited to: weather, teacher absence, or governmental restrictions or advisory. You are responsible for having compatible technology, internet access for streaming, and subscribing to ISB’s digital class platform/app. Virtual class schedules and access codes will be provided by ISB.
REFUND POLICY
Tuition, membership fees, recital fees and costume fees are not refundable for any reason. No refunds will be given if a similar online class is provided in place of an in-studio class.
MAKE-UP CLASS POLICY
Because of our strict student to teacher ratio and current health restrictions, if a student misses an in-person class, an in-person make-up class will be offered only if the classroom is not at maximum capacity. Otherwise, make-ups for in-person classes will have to be taken online, and no prorated tuition or refund will be issued. Missed classes will be made up in one of our available classes. It is the student’s responsibility to make up the missed classes by attending a virtual class within 30 days of the absence, or the class will be considered forfeited. WITHDRAWAL PROCEDURE:
ISB has a 30-DAY CANCELLATION POLICY. Authorized parent or guardian must submit a Withdrawal Request Form via email or at the front office at least 30 days before the student is to be withdrawn. Withdrawal shall be effective, and a student shall be considered withdrawn, at the end of the 30-day period following notice of withdrawal. Tuition for the 30-day period prior to the withdrawal effective date will be charged whether the student attends classes or not.
If there is an outstanding balance on the student’s account for tuition, fees, costumes, or any other expense, payment is due with the Withdrawal Request, and may be offset by any prepaid tuition.
Verbal, text, or email drop requests that are not on a Withdrawal Request Form will not be honored. A Withdrawal Request Form may be obtained by email to admin@inspireballet.com
You are responsible for payment for your student's classes whether or not your student attends class and until the withdrawal effective date (30 days after submitting a signed version of the official Withdrawal Form). If a student stops coming to class without notification by a Withdrawal Request From, that student's account will continue to be charged. Inspire School of Ballet does not offer refunds for non-attendance without proper Withdrawal.
CLASSROOM POLICIES
Gum chewing is not allowed in the ISB studios.
Dance shoes should not be worn outside. You will not only ruin your shoes, but our dance floors could be damaged as well. Additionally, street shoes should not be worn on dance floors. Please leave street shoes in the reception area cubbies.
All shoes, attire and other items should be labeled with the student’s name. ISB is not responsible for lost or stolen items. Please do not bring any valuables to class.
No food allowed in the studios. Water is OK everywhere.
If 2 or less students attend a class, the class will be shortened to 30 minutes.
Instructors are subject to change at any time.
Classroom policies and dress code apply to virtual class participation also.
TARDINESS
Arriving on time or a few minutes early is important. Students are expected to be in class, ready to dance at class start time. A student arriving more than 15 minutes late may be asked by the teacher to sit and observe because they missed a valuable part of their warm up and are more susceptible to injury if they dance without a proper warm up. Observing class still counts as attending class and does not qualify for a make-up class. Students who are consistently late for class may be removed from a class or performance.
STUDENT PICK-UP AND DROP-OFF POLICIES
For student safety, parents must walk their child to the studio and remain with their student until the class begins.
Please do not allow children to loiter outside the building or in the parking lot.
Please pick up your child in a timely fashion as ISB is not responsible for students left unattended. For student safety, parents must walk in and pick up their children. Children may not go out into the parking lot without adult supervision. If a parent is consistently late in picking up his/her child, he/she will first receive a written warning. Subsequent late pick-ups may result in removal from a class or performance.
STUDENT CODE OF CONDUCT
• Students will respect all parents, teachers, staff and fellow students at all times.
• Students will abide by the dress code. No jewelry allowed (except small stud earrings).
• Students will not talk in class unless asked to do so by their instructor.
• Students will use appropriate language and behavior in the studio.
• Students will observe all health and safety instructions given by teachers and staff.
If an instructor determines that a student should no longer participate in instruction due to unacceptable behavior, the parents will be notified, and Inspire School of Ballet has the right to refuse further instruction.
Inspire School of Ballet reserves the right to refuse or decline service at any time.
PARENT CODE OF CONDUCT
• Parents will support teamwork and create a positive studio environment.
• Parents will not make any disparaging remarks of any type regarding teachers, choreographers, choreography placement, costumes, other parents, other schools, or publicly criticize them in the studio, online, or on social media.
• Parents will not make any negative remarks regarding other parents or their children in the studio.
• Parents with questions or concerns will direct all questions to the ISB staff. If you have a concern/issue, please set up an appointment to discuss it in private.
• Parents will keep all conversations in the studio appropriate.

Presidents Day
Spring Break (1 week)
Memorial Day (May 24-27th 2024)
Note: Tuition payments are not affected by holidays.
RELEASE FORMS:
I release Inspire School of Ballet LLC, its officers, directors, staff, employees, and independent contractors, volunteer helpers, and landlords from any and all liability that may result from myself, my children, or any member of my family participating in dance lessons, exercise classes, rehearsals, parties, private lessons, performances, field trips, preschool classes, or any function sponsored by or located at Inspire School of Ballet.
I understand and agree that in participating in any class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of ISB classes, rehearsals, performances, or activities. I also exempt, release, and indemnify ISB, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by ISB.
I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Inspire School of Ballet, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my/my child’s physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
I agree to hold ISB, its officers, directors, staff, employees and independent contractors, volunteer helpers, and landlords 100% harmless for any and all injury that may result from my child, myself, or any member of my family participating in the activities listed above. Our participation is completely voluntary.
I understand that I am solely responsible for obtaining any necessary health treatment, providing insurance coverage, and making payment for services related to any injury that may occur by participation in any activity at ISB.
PHOTO RELEASE: Permission is granted to ISB to use photographs and video footage of students for publicity purposes.
COVID-19 RELEASE OF LIABILITY
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending programs at Inspire School of Ballet (ISB) and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at ISB may result from the actions, omissions, or negligence of myself and others, including, but not limited to, ISB employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at ISB or participation in ISB programming (“Claimsâ€). In addition, I agree to follow COVID-19 both CDC and ISB standards and guidelines. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless to ISB, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of ISB, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any ISB program.
Thank you for evaluating your child DAILY before coming to the Academy. Has your student had any of the following respiratory symptoms? Cough, Shortness of breath, OR at least TWO of these symptoms: Fever, Chills, Repeated shaking with chills, Muscle pain, Headache, Sore throat, New loss of taste or smell, Vomiting, Diarrhea. Is there an ill family member in your home? If you answer YES to any of these questions, please keep your child home and schedule a makeup class. By sending your student to Inspire School of Ballet, you are agreeing that your child has not had any symptoms of the virus, nor been in contact with anyone with COVID-19 and will abide by all the rules and regulations implemented to protect all the students.
