I have received or will obtain a copy of Aledo Academy of Ballet policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Aledo Academy of Ballet does not give credit and/or refunds for class(es) missed due to holidays, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Aledo Academy of Ballet, and I voluntarily assume the risks associated with such participation.
The novel Coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregations of groups of people.
Aledo Academy of Ballet (AAB) has put in place preventative measures to reduce the spread of COVID-19; however, AAB cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending AAB could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending AAB and that such exposure of infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at AAB may result from the actions, omissions, or negligence of myself and others, including, but not limited to, AAB employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at AAB or participation in AAB classes. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless AAB, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of AAB, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in AAB program, class, or event.
COMMUNICATION The studio’s primary form of communication is email. Be sure to provide the studio a primary
email address for studio updates and monthly e-newsletter publications. Parents also may like our Aledo Academy of
Ballet Facebook page and request to join our Aledo Academy of Ballet Parent Group on Facebook to receive the latest
studio news such as weather closings.
DROP-OFF ONLY The lobby will remain closed for the 2021-2022 School Year, parents are asked to drop students
off at the door. In exchange two Parent Watch Weeks will be scheduled for you to watch your student in class. Masks will
remain optional for the 2021-2022 School Year.
MONTHLY TUITION Monthly tuition will be auto drafted on the first of each month. There will be a $10 processing
fee for any payments that are unable to be processed by the DSP system.
CLASS ATTIRE Students are required to wear their class dress code leotard, pink tights and proper shoes in class. To
prevent damage to the special dance flooring, dance shoes should not be worn outside the studio. Street clothes or
appropriate cover-ups must be worn when outside the studio. Sandals should be slipped over ballet shoes before students
HAIR Hair is to be worn in a ballet bun for all dance classes. Short hair must be pinned away from the face. No
ponytails, please. Bangs must be pinned back.
ATTENDANCE/ABSENCES In the event of an absence, please notify the studio by phone or email as soon as possible.
With advanced notice, make-up classes can be arranged with Miss Caroline over email.
TARDY POLICY Students who are late to class should wait by the studio door for the teacher’s invitation to join the
RECITAL & COSTUME FEE A Spring Recital and Costume fee will be due by January 8, 2022. If a student enrolls
after January 8, 2022, the costume/recital fee will be due at time of registration.
PHOTO RELEASE WAIVER I hereby grant, Aledo Academy of Ballet, permission to use my child’s likeness in a
photograph, video, or other digital media in any and all of its publications, including web-based publications, without
payment or other consideration.
LEGAL RELEASE By signing this release, I acknowledge the academy policies and understand that my child(ren)
is taking classes at their own risk and I agree to hold Aledo Academy of Ballet, its instructors, and/or assignees harmless
for any liability, claims on demands for personal injuries, sickness or death as well as for expenses due to loss or damage
to personal property while on or around the premises. By signing this form, I agree that all tuition and related fees are not
refundable and not transferable.
2021-2022 ALEDO ACADEMY OF BALLET STUDENT HANDBOOK I have read the student handbook and our family
will adhere to all policies and procedures contained in the handbook.
