I acknowledge that I have voluntarily applied to participate in dancing instruction, training and performances at Studio A Dance Academy LLC. I am aware that participating in these activities (Ballet, Pointe, Lyrical, Contemporary, Tap, Jazz, Hip Hop, Tumbling, Aerobic, Yoga, and Performances) can result in injuries, and I am voluntarily participating in these activities with the understanding of the danger involved and I hereby agree to accept any and all risks of injury. As lawful consideration for being allowed by Studio A Dance Academy LLC to participate in these activities and to use their facilities, I hereby agree that I will not make a claim against, sue, attach the property of, or prosecute Studio A Dance Academy LLC, any of their affiliated organizations, owners, or employees for injury or damage resulting from the negligence or other acts, however so caused by any employee, owner, agent or contractor of Studio A Dance Academy LLC as a result of my participating in dance activities. In addition, I hereby release and discharge Studio A Dance Academy LLC, all of its instructors, owners, employees, heirs, guardians, legal representatives, or assigns now have or may have hereafter from all actions, claims, or demand for injury or damage resulting in my participation in dance classes, instruction, or performances.

In the event that I cannot be reached, I hereby give my permission to the management, faculty, staff, and chaperones of Studio A Dance Academy LLC to authorize any emergency medical care that may be required by the above student during participation in classes, performances, or any related Studio A Dance Academy LLC events. This authorization extends throughout the current academic year and throughout the summer or until the student is no longer enrolled at Studio A Dance Academy LLC, whichever comes first. I understand that I am responsible for any and all charges as a result of such care or medical treatment. I understand that at no time will Studio A Dance Academy LLC faculty drive an ill or injured student to an emergency facility.

I hereby authorize Studio A Dance Academy LLC to record the student's picture and voice on photographs, films, and video, to edit these recordings at its discretion, and to incorporate these recordings into movie and sound films on video, radio, television broadcast programs, CD's, flash drives, DVD's, and digital media. I also give my permission for Studio A Dance Academy LLC to use and license others to use these materials for publicity, advertising and sales promotion and to use the student's name, likeness, and voice and biographic or other information in connection with them. I acknowledge no promises of compensation were made by Studio A Dance Academy LLC for such use.

I agree to pay Studio A Dance Academy LLC for the dance instruction of the above student at the agreed rate per class per student per month for the student's period of study. I understand I can make payment online, by check (payable to Studio A) or cash, and that there will be a $30 charge for returned checks.

I understand that payment of tuition entitles students to take those dance classes scheduled and that no refunds are given for classes missed because of illness, vacation or closings due to inclement weather.

Starting September, I understand all tuition fees are due the 1st of every month; that if payment has not been received by the 10th of the month, a late fee of $10 per month will be assessed. A $15 registration fee is due upon registering.
