While enrolled in classes at Center Stage, my child(ren) and I agree to:

Abide by the rules of Center Stage Dance studio, and the rules of Trinity Nazarene Church
Show respect for Center Stage staff, including volunteers and assistants
Show respect and kindness to fellow students and parents
Refrain from bullying, harming, threatening, or humiliating anyone at the studio
Refrain from using foul language in class
Refrain from any inappropriate physical contact with other students, and from romantic or sexual displays of affection while on the premises, and during any activity with Center Stage
Refrain from the use of alcohol, smoking of any kind, and drug use while on the premises, and during any activity with Center Stage
Refrain from using parts of the building that are off limits during classes. This includes the Children's Church area and it's attached rooms, and the downstairs portion of the church building
Dress appropriately for class, according to the provided dress code
Refrain from bringing and using electronics in the studio during classes (cell phones, tablets, phone watches, music players, etc)
Pay tuition on time at the beginning of each month, September thru May. I understand that repeated failure to pay on time may result in the termination of my enrollment
Bring concerns to the teachers' attention in a kind and understanding manner, and at an appropriate time. I will not interrupt a teacher's class to speak with them unless it is an emergency
Be on time to classes, and pick up my child(ren) on time after classes are finished
Supervise my child(ren) while not in classes on the premises

Attendance: A total of 8 absences for the year are allowed. It’s up to you to keep track of your child’s absences. After more than 8 absences, the student will not have the opportunity to promote to the next dance level the following year, or to participate in our yearly ballet (this is a separate show from recital.) This will also affect their ability to participate in the dance recital. This applies to missing recital without prior notice. Students who are 15 minutes late or more to class will be marked absent. (If an ongoing illness or family matter causes multiple absences, we'll be glad to discuss this with you and make provisions). 8 is the absolute maximum, and we do not like for students to miss this many classes.

Dress code: The dress code for each class can be found by clicking on your child's class, and going to the "description" section. Please dress out properly for every class, with appropriate shoes and hair.
Payments: Tuition is due the first full week of each month, no exceptions. A fee of $10 will be applied to late payments. Please be considerate and pay on time. Repeated late payments will result in termination of enrollment at Center Stage.

Drop off and pickup:
Students ages 5 and under must have a parent present during all classes. Students ages 6 and over may be dropped off at the start of classes, and picked up at the end, however they cannot be unsupervised before or after classes. Students ages 10 and over may be dropped off and use the waiting room between classes, without an adult present. This is at your own risk.
General: Students should use the bathroom before class begins, and be prepared to start class on time. No fixing hair, tying shoes, changing clothes etc. during class. Student need to bring their own water bottle to class.

Building rules: Trinity Nazarene Church, where our classes are held, has been very generous in allowing us to rent space from them. Center Stage does not own any part of the building, we only rent space from them. Please respect their rules, and use only the parts of the building designated for dance: The waiting area and the studio spaces. The adjacent children’s church rooms and the downstairs portion of the building are off limits unless otherwise specified. Please discuss this with your kids, and with anyone who brings your children to dance.

Behavior: By enrolling at Center Stage, you agree to our behavior policies (see Behavior Agreement page). Students who do not follow these rules will have the following consequences:

1st incident- A message will be sent to the parent/guardian informing them of the issue
2nd incident- Student must give an apology to the teacher
3rd incident- 1 week suspension from dance classes
4th incident- Student will not be allowed to continue taking classes
Disclaimer: If a student is found doing drugs, smoking, drinking, or physically hurting others while at Center Stage, they will be asked to leave immediately and will not be allowed to re-enroll.

Center Stage Dance Studio & Trinity Nazarene Church 3515 Shackelford Rd. • Florissant, MO 63031 Hold Harmless Agreement 2024-2025 Fall- Spring Season, September 2024-May 2025, June 2025 for recital.

In consideration for my child being permitted to participate in the Center Stage Dance Studio program (referred to as “the studio” for the rest of this document),
I/We (parents, legal guardians, and students) agree to the following provisions:

Acknowledgment of Risks: I/We understand that there are numerous risks associated with participating in dance activities, including those risks present during classes and activities, on the premises before, during, and after various classes and events, as well as in travel to and from the Studio and various venues of performance. Some of the specific risks include the placement of unusual stresses on the body, falls and tripping which may lead to accidents resulting in, but not limited to, mild to severe bodily injury. I/We understand that the Studio cannot be responsible for any injuries or damages experienced by my/our child during her/his/their participation in such activities. I/We understand this acknowledgment of risk extends to use of the facilities by me/us or anyone I/we bring along with me/us, either during classes or before and after. Anyone using the facilities are under my/our care during their stay in any part of the building.
Medical Care & Parent Responsibility: Center Stage Dance Studio and/or Trinity Nazarene Church are not authorized to provide medical attention to participants of the studio. Should a medical emergency arise, it is the sole responsibility of the parent/guardian who must remain available during all classes to provide any medical care. If the parent/guardian are not available, the studio is not liable for it’s chosen course of action. By signing below, you also agree to pay all fees associated with dance classes, and to pick your child/children up on time after each class. Failure to comply with pick up policies may result in termination of your enrollment at Center Stage.

I further agree to indemnify and hold harmless and defend Elizabeth Pietrzak, Brittany Lerdahl, Emily Scott, Emily Dixon, Center Stage Dance Studio, Trinity Nazarene Church, its teachers, officers, agents, and employees from injuries, damages and losses sustained by me or my child arising out of, connected with, or in any way associated with the activities of the program(s). I have read and fully understand the above program details and waiver and release of all claims.
