You must be over the age of 18 to register for an account.

Tuition payments are due no later than the 15th of each month. A $15 late fee will be added to payments made after the 15th of each month.

In the case of a medical emergency, I hereby give permission to trained professionals to administer emergency medical treatment to my child (or myself) should an emergency arise in my absence.

28th Avenue Dance Studio Registration Fee Waiver-
I, the undersigned, acknowledge and agree to the following:

Annual Registration Fee:
I understand that upon registering for a dance class at 28th Avenue Dance Studio, an annual non-refundable registration fee of $15.00 is required.

I certify that my child (or myself) is in proper physical condition to participate in dance, tumbling, and exercise classes at the 28th Avenue Dance Studio. I understand that there are potential risks involved in participating in such activities and I agree to assume all risks of injury or damage that may occur from my child's (or my own) participation in all activities at the 28th Avenue Dance Studio.

I hereby release and hold harmless Oran, Inc. (doing business as 28th Avenue Dance Studio), the owners, instructors, employees, and volunteers from any liability, loss, harm, cost, or damage sustained as a result of my child's (or my own) participation in dance, tumbling, and exercise classes, performances, rehearsals, and recitals.

We sometimes use pictures/video from classes or performances to promote the studio on our Facebook page or Instagram. Please accept this policy if we have your permission to post pictures/video of your child (or of yourself) on social media to promote the studio.
