In order to register your dancer a valid debit/credit or checking account must be placed on file. Tuition is due on the first of every month.
If your dancer will no longer be attending classes a withdraw form MUST be completed and submitted back to the office. If a withdraw form is not submitted your dancers tuition will still be processed on the first of every month.

ALL ACCOUNTS WILL BE ‘AUTOBILLED’ ON THE 1st OF THE MONTH. This will cover all extra charges for the previous month (vending, dancewear, etc) and the tuition for the current month. A receipt will be emailed to the primary email on the account. (THERE IS NO ADDITIONAL FEE FOR THIS SERVICE). REQUIRED: CREDIT CARD OR BANK PLACED ON DANCE ACCOUNT FOR TUITION AUTOBILL.

If your card declines, you will have until the 5th of each month to provide an alternative payment. On the 6th, we will add a $35 late fee. Failure to provide payment by the 1st of the following month, will result in students not being able to attend class or perform in year-end recitals. A $5.00 late fee will be added every 5 days after the 5th day account is late.

Your card will continue to be billed, unless we receive a “Withdrawal” form to remove students from class roster prior to the 1st of the month. NO EXCEPTIONS! Failure to turn in a “Withdrawal” form will result in continued tuition charges & late fees, until we receive a completed form or letter. Any outstanding fees on the account must be paid in full upon withdrawal.

TUITION REMAINS THE SAME EACH MONTH REGARDLESS OF WHETHER IT IS A 5-WEEK MONTH OR A SHORTENED MONTH DUE TO HOLIDAYS. Tuition is not prorated, refunded or credited for missed classes or holidays. Any student who misses a class may make-up classes in other technique classes comparable for their age/skill level after signing up through the front desk.

If your child is absent due to any reason (e.g. illness, vacation, or other activities) payment is still run on the 1st to continue to hold the spot in class for the student. There are NO REFUNDS for missed classes. Makeup classes are available upon request

A $35 return check fee will be added on all insufficient fund checks.

If parents are splitting payment for their student(s), written consent must be given from BOTH parents as to who is paying what portion of the charges. Any changes that will affect payment must be approved by both parties, who must each inform our office manager of such changes. It is the responsibility of the parents to figure out payment plans, and not that of the BC DANCE staff.

All accounts must be completely paid off before June 1st for students to be able to participate in the end-of-the year recital.

BC needs a 30 day notice if you decide you would not like to continue the following month. Failure to do so will result in the full tuition amount to be billed.

If you decide to take time off, the re-registration fee within the season is $15.

IN CONSIDERATION of being allowed to participate in any way in any BC Dance class, team, competition, recital or event under the auspices of The Ballet Conservatory of Dance, Music & Art LLC, I, the undersigned, acknowledge, appreciate, and agree that:

I RISK BODILY INJURY, INCLUDING PARALYSIS, DISMEMBERMENT, DISABILITY AND DEATH, AND while particular rules of dance, skills, equipment, and personal discipline may reduce this risk, THIS RISK OF SERIOUS INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO OR LOSS OF PROPERTY; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS, and assume full responsibility for my participation; and
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and remove myself from participation; and
I, FOR MYSELF, and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, HOLD HARMLESS and PROMISE NOT TO SUE THE BALLET CONSERVATORY OF DANCE, MUSIC & ART LLC (dba BC DANCE), their officers, officials, volunteers, employees, agents, and/or other participants, sponsors, advertisers, and, if applicable, the owners and lessors of premises used for the activity (“RELEASES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, and/or LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIGENCE OR WANTON MISCONDUCT, to the fullest extent permitted by law.

We, the staff of BC DANCE, recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver, you release BD DANCE and all its employees from all claims on account of any injury which may be sustained by your child while attending any dance class, event associated with BC Dance, or outside performance. You also affirm you now have, and will continue to carry, proper primary medical, health, and hospitalization and accident insurance, which you consider adequate for the protection of both your child and BC Dance.

I have read this Release of Liability and Waiver Agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without inducement.
