We realize that participation in dance and activities can involve personal injury. Despite precautions, accidents and injuries occur. By signing this release form we assume all risk related to the use of any and all spaces at Edge Studio. We agree to release and hold harmless Edge Studio including it's owners, teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. We will not hold Edge Studio liable for any personal injury or any personal property damage, which may occur on the premises before, during or after class. Furthermore, we agree to obey the class and facility rules and take full responsibility for our behavior in addition to any damage we may cause to the facility.

We agree to allow Edge Studio of Dance to photograph or videotape my child at any Edge Studio event related to the use of any and all spaces used by Edge Studio for the use of advertising, social media and literature for Edge Studio. This is included but not limited to in the studio, recital, workshops, conventions, and fundraisers. No compensation will be allotted for the use of these photographs or videotapes.

Edge Studio values our families and students. In order to provide the best possible service, we must insist on prompt payment. If an account falls behind more than 2 months, we will have to ask that the student observe class and not participate until the account is paid in full. If you sign up for auto pay this will cover all balances.

A mandatory, non-refundable recital media fee and costume fees will automatically be posted to your account. Costume fees are due December 1st, and the recital media fee is due in May. Please email us by December 1st if your child is not participating in the recital.
