Studio One (TMJ, LLC) Liability Waiver

I understand and acknowledge that participation in dance classes, workshops, performances, and any related activities involves inherent risks and hazards. I voluntarily choose to participate in these activities at Studio One (TMJ, LLC) and hereby assume all risks associated with such participation.

Release of Liability: In consideration for being permitted to participate in the dance classes and activities at the Studio, I hereby release, waive, discharge, and covenant not to sue Studio One (TMJ, LLC), its owners, employees, instructors, volunteers, and agents (collectively referred to as "Released Parties") from any and all liability, claims, demands, actions, causes of action, costs, and expenses, whether at law or in equity, arising out of or in any way related to my dancer's participation in the dance classes, workshops, performances, and any related activities, including, but not limited to, personal injury, illness, property damage, or death, even if caused by the negligence or fault of the Released Parties.

Assumption of Risk: I understand and acknowledge that dance activities involve physical exertion and may require movement and contact with other participants and objects. I am fully aware that there are risks, dangers, and hazards associated with dance activities, which may result in bodily injury, illness, or property damage. I voluntarily assume all risks and hazards associated with my dancer's participation in dance classes and activities at Studio One (TMJ, LLC).

Medical Considerations: I certify that my dancer is in good physical condition and have no medical conditions, injuries, or disabilities that would prevent or limit his or her participation in dance classes and activities. I understand and agree that it is my responsibility to consult with a medical professional before allowing my dancer to participate in any dance activities if I have any concerns regarding his or her physical or mental health.

Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all liabilities, claims, demands, actions, causes of action, costs, and expenses, including reasonable attorney fees, arising out of or in any way related to my participation in dance classes, workshops, performances, and any related activities.

Severability: If any provision of this liability waiver is found to be unenforceable or invalid under any applicable law, the remaining provisions shall continue in full force and effect.

I have read this liability waiver, fully understand its terms, and voluntarily sign it without any inducement or assurance of any nature. I acknowledge that I am 18 years of age or older and have the legal capacity to enter into this agreement. If the participant is under 18 years of age, a parent or legal guardian must sign or accept here.

I grant permission to Studio One (TMJ, LLC) to use photographs or videos of my child for promotional and educational purposes. I understand that these materials may be used in various media formats, including but not limited to print, online, social media, and video for the purpose of promoting the Studio's activities and showcasing my child's participation in dance classes, workshops, and performances. I acknowledge that I have read and understood the terms of this consent and agree to release the Studio from any claims or liabilities associated with the use of my child's image or likeness as described herein.
