GENERAL TERMS
1. I am the authorised parent/guardian of the child I have registered to attend classes and I agree that any activities in which he/she participates is entirely at his/her own risk. In the event of an accident or illness, I authorise the obtaining on my behalf medical assistance as required. **Please note that you must email us with any relevant medical conditions**
2. Enrolment is presumed for the entirety of the year. I understand that fees will continue to be charged until LKM Performing Arts is notified in writing of withdrawal from classes.
3. To withdraw from a class, two weeks notice from scheduled class day must be provided in writing to accounts@lkmdance.com.au. Normal class fees will be charged up to that date.
4. I give permission for the Studio to display, print, copy and publish in LKM Performing Arts’ brochures, flyers, newsletters, website, video and other promotional material, photos or video footage of my child taken during dance activities and performances.
5. I agree that even if my child does not attend some classes, full fees must still be paid (a makeup class may be organised for missed classes if appropriate).
6. Costume hire / purchase fees are charged in two instalments over terms 2 and 3. For students commencing after term 2, the full amount will be charged in term 3. This fee is strictly non-refundable.
7. The Concert Performer Pack fee will be charged in term 4 along with your class fees. This fee is strictly non-refundable.
8. All fees must be paid in full for students to perform in the mid year and end of year concerts or other events. This includes costume and performer pack fees.
9. ACCOUNT QUERY: if you have a query on your account, please email our accounts office on accounts@lkmdance.com.au BEFORE the payment is due.
10. LKM Performing Arts reserves the right to pass any outstanding fees onto a Debt Collection Agency, this will incur a 20% surcharge on the balance of the account and any related fees will be incurred by the client.
11. PRIVACY STATEMENT: LKM Dance Studios will not disclose any of your personal details to third parties without your permission. All information is kept strictly confidential.
DIRECT DEBIT PAYMENT TERMS (AutoPay)
LKM Performing Arts’ payment partner for direct debits is Ezipay. By signing up for “AutoPayâ€, I agree to the following:
• I will keep my payment details (bank account or credit card details) updated through my Parent Portal.
• I authorise LKM Performing Arts to process the direct debit via Ezipay as per the current year's payment dates.
• Any additional fees, i.e., costume hire, concert performer pack, troupe fees etc., will be included in the direct debit. I understand that these fees are strictly non-refundable.
• I agree to pay the associated fees when paying by direct debit with Ezipay – Visa or Mastercard $0.44 + 1.98%; bank account (BSB & a/c) $0.99.
• Should a payment dishonour, I will be charged $21.90 dishonour fee from Ezidebit.
• If my direct debit fails in 2 consecutive payments, students will be unable to participate in classes until the outstanding amount is paid in full.
• FAMILY & MULTIPLE CLASS DISCOUNT: you are only eligible for the discount if your account is paid on time. Late payments will forego the discount and your account will be adjusted and a new invoice issued without the discount.
• OVERDUE ACCOUNTS WITH NO ARRANGEMENTS IN PLACE: If accounts remain outstanding, with no contact or arrangements in place, the matter will be passed on to a collection agency and any fees associated with this will be passed on to you.
PAY IN FULL AT THE BEGINNING OF TERM PAYMENT TERMS
• I agree to pay the total term fees in full by the due dates as advised by LKM Performing Arts.
• Payment options are: online through your Parent Portal (PayPal or Ezidebit), bank transfer to our account, cash, credit card or EFTPOS (available at Springwood). I agree to pay any associated fees for these transactions.
• LATE PAYMENT FEE: a $25.00 late payment fee will be added to your account on every invoice that is not paid by the due date if you have not contacted us prior to the due date. Students will not be allowed to participate in classes if your account remains unpaid and no arrangements have been made.
• UNABLE TO PAY BY THE DUE DATE: we understand that families can sometimes have difficulty paying on time. If this happens, you should email accounts@lkmdance.com.au BEFORE the due date to discuss payment arrangements.
