I acknowledge and understand the nature of physical activities, I understand accidents and/or physical injury may occur. I understand there is a risk of illness. I agree to participate at my own risk. I agree to release and hold harmless All Parties Involved at the Jean Kettell Studio of Dance LLC, located at 7014 Holabird Avenue, and in other venues where I may participate in studio activities at other physical locations. This waiver includes any and all staff members from any cause of action, claims, or demands now and in the future. I will not hold Jean Kettell Studio of Dance LLC, as a whole or its staff liable for any personal injury, catastrophic injury or illness or any personal property damage or loss, which may occur on the premises before, during or after classes. I am agreeing to this for both myself and minor children.

I understand that both video and photographs may be taken while participating but not limited to events, classes, and performances with Jean Kettell Studio of Dance LLC.I understand that the images may be used in internet, print or video presentations.Some materials may be used to promote Jean Kettell Studio of Dance LLC on Facebook, Instagram, the Jean Kettell website, etc. I understand that I will not be compensated for the use of my image. I agree to this form for myself and my minor child. This agreement will remain in effect until rescinded in writing.
