I also hereby assign and grant to Sugarloaf Performing Arts, Sugarloaf Youth Ballet, and Gwinnett Ballet Theatre as well as their authorized representatives, the right and permission to use and publish the photographs or electronic representations and/or sound recordings made of me or my child at all dance activities. I further authorize the reproduction, copyright, exhibit, broadcast, electronic storage, and/or distribution of said photographs, film, videotapes, electronic representations and/or sound recordings without limitation at the discretion of Sugarloaf Performing Arts, and I specifically waive any right to any compensation I may have for any of the foregoing.

Important Note: If you are not pleased with a published photograph or electronic representation please inform us and we will take actions to remove it.

I do hereby release Sugarloaf Performing Arts, Sugarloaf Youth Ballet, Gwinnett Ballet Theatre and all employees, volunteers, and related parties from any claims for damages, injuries or loss of property on or around facility premises, or at any function held at another location in connection with the above listed entities. I declare that the student registered is in good health. Attendance at and participation in these activities indicates the complete acceptance of such risks by the performers and parents. I have carefully considered the risk involved and hereby give my informed consent for my child to participate in all activities offered in the program.

Please be advised that it is the parent’s responsibility to supervise children before and after class. We are not responsible for supervising children after their class ends. Please pick them up promptly after class. A late pick-up fee of $5 may be assessed for the first 10 minutes late and $10 for each 15 minutes thereafter for all students remaining after their designated class time.

Tuition is a yearly fee; it has been broken down into nine monthly payments for your convenience. A $15 late fee is incurred after the 10th of the month. Tuition does not change with studio closings or holidays. It is understood that our program runs with the school year and ends with the recital. If you are dropping for any reason, you must advise us in writing by the 1st of the month prior to the month you intend to drop (ie. if you intend for your student to be removed from classes October 1st, we must be notified of your intention to drop by September 1st in order to avoid paying October tuition). Failure to do so will result in your having to pay for one additional month’s tuition, even if your child did not attend classes.

I understand that participation in recital is required for all dance classes at Sugarloaf Performing Arts.

I accept Sugarloaf Performing Arts studio fee outline and studio policies.

I understand that participation in dance activities involves the risk of personal injury, including death, due to the physical, mental, and emotional challenges in the activities offered. I also understand that participation in these activities is entirely voluntary and requires participants to follow instructions and abide by all applicable rules and the standards of conduct. In case of an emergency involving me or my child, I understand that efforts will be made to contact the individual listed as the emergency contact person by the medical provider and/or adult leader. In the event that this person cannot be reached, permission is hereby given to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medication for me or my child. With appreciation of the dangers and risks associated with programs and activities, on my own behalf and/or on behalf of my child, I hereby fully and completely release and waive any and all claims for personal injury, death, or loss that may arise against the Sugarloaf Performing Arts, Sugarloaf Youth Ballet, Gwinnett Ballet Theatre, and all employees, volunteers, related parties, or other organizations associated with any program or activity.
