Release of Liability
I/we acknowledge that participation in dance classes and activities carries inherent risks of personal injury. Despite the precautions taken, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks associated with the use of any and all spaces utilized by MORE2Enterprises LLC, doing business as MORE2Dance. I/we hereby release and hold harmless MORE2Enterprises LLC, its owners, teachers, dancers, staff members, and the facilities used by both entities from any and all causes of action, claims, or demands, present or future. I agree to allow the teaching staff or representatives of MORE2Enterprises LLC to be alone with my child. I/we absolve MORE2Enterprises LLC, doing business as MORE2Dance, of any liability for personal injury or property damage that may occur on the premises before, during, or after classes. Furthermore, I/we agree to abide by the class and facility rules and accept full responsibility for our behavior, as well as any damage caused to the facilities utilized by MORE2Enterprises LLC, doing business as MORE2Dance.
Medical Emergency Policy
I grant permission to the staff of MORE2Enterprises, doing business as MORE2Dance, to take necessary first aid or emergency measures for the care and protection of my child while under the studio's supervision. In the event of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if deemed necessary by the emergency unit. I acknowledge that in certain medical situations, the staff may need to contact the emergency resource before reaching the child's parent, physician, or other person acting on the parent's behalf. I also understand and agree that the child's parents or legal guardians will be responsible for any expenses incurred during such emergency situations.
