I,the undersigned parent/guardian of the participant listed, do hereby give permission for him/her to participate in dance classes at Miss Autumn's Dance Class. I understand that there is the possibility of injury. I hereby waive, release and forever discharge any and all rights and claims for damages which may arise now or in the future against any Dance Class representative. I authorize Dance Class to use their best judgement in any emergency.
I hereby give permission for my child to be used in photographs and/or videos for promotional and marketing purposes of Miss Autumn's Dance Class.
Dear MADC Families,
We hope this message finds you and your loved ones in good health and high spirits. At Miss Autumn’s Dance Class, we greatly appreciate your commitment to our studio and the art of dance. We understand that life can get busy, and sometimes, deadlines can slip our minds. To maintain the quality of instruction, facilities, and programs we offer, it is essential that we maintain a consistent and timely tuition payment schedule.
Effective 9/5/23, we are implementing a friendly, yet firm, Late Tuition Policy to ensure that tuition is paid in full and to keep our operations running smoothly. Please take a moment to familiarize yourself with the following guidelines:
Monthly Tuition Due Dates:
Tuition for the upcoming month is due on the 1st of each month.
Families signed up for auto-pay will have tuition withdrawn from their account on the 1st of each month.
Tuition is posted on everyone's account on the 1st of each month.
Late Payment Grace Period:
We understand that life can be unpredictable, so we provide a grace period until the 10th of each month to submit your payment without any consequences. This grace period allows you some flexibility in case of unexpected circumstances.
Consequences for Late Payments:
If payment is not received by the 15th of the month, A late fee of $10 will be added to your account to cover administrative costs.
Suspension of Classes:
If payment is not received by the 15th of the month, your child will not be able to attend classes until the account is brought up to date. We do this to maintain fairness to all families who have paid promptly.
If tuition remains unpaid for two consecutive months (i.e., if payment is not received for the current month and the previous month), we reserve the right to unenroll your child from their dance classes. To re-enroll, all outstanding balances, late fees, and the current month's tuition must be paid in full.
We understand that financial difficulties can arise. If you are facing difficulties making payments, please reach out to our administrative team as soon as possible. We are committed to working with you to find a solution that allows your child to continue their dance education.
Our goal is to ensure that every child can continue to enjoy the benefits of dance education at Miss Autumn’s Dance Class . We believe that with open communication and adherence to these policies, we can continue to provide an exceptional dance experience for all our students.
