ACKNOWLEDGEMENT OF RISK, WAIVER OF LIABILITY, MEDICAL RELEASE,PLEASE READ BEFORE SIGNING BELOW!!!
_______________and_________________ parents of ___________________, do hereby represent to DANCE EXPRESSIONS that their child is of sound health and has no history of a medical condition (or physical) which could in any shape, manner of form place the child at risk because of the said condition. The said parents also hereby acknowledge that they have been informed by DANCE EXPRESSIONS of the nature of the instruction that their child will receive and that such instruction involves physical exercise and stress. It is further understood that DANCE EXPRESSIONS does not carry medical insurance for it's students. Each student is required to be covered by their own family's medical policy and if any injury occurs it is understood that the student's own policy is their only sour of reimbursement.
A $25.00 registration fee is required of all new and returning students. Class space is not held until the registration fee is paid. A $50 costume deposit is also due at the time of registration. All fees are non-refundable!!!
PHOTO RELEASE: Dance Expressions is hereby granted permission to take photos of the students to use in brochures, websites, posters, advertisements and other promotional materials. Permission is granted for DANCE EXPRESSIONS to copyright such photographs in its name.

Studio Policies and Procedures
Registration: A non-refundable registration fee of $25 is due at the time of registration to secure a place in the classes available. Class placement is determined by the ability of the student and age is also taken into consideration. Students may need to be shifted to another class if their teacher feels it is necessary.
Costume Deposit: There is a costume deposit fee of $50 due at registration. This is a non-refundable fee and goes towards costumes for the annual recital in June. Parents are required to pay the balance for costumes in full.
Tuition: Tuition is based on a 9 1/2 month school calendar with the dance year beginning after Labor Day and ending after the recital at the year end (usually the first weekend in June). Tuition is always due by the Ist of each month and late on the 5th of the month. A late fee of $15.00 will be added if payment is not received by the 5th. There are NO REFUNDS given for missed classes. Students may make up missed classes during a class similar to theirs. All payments are

Returned Checks: A fee of $25.00 will be added to the account for any checks returned from the bank for any reason.
Withdrawal/Drop Policy: To withdraw from a class the parent/guardian must inform Mrs.
Angie in person. NO WRITTEN NOTIFICATIONS will be accepted. Dance Expressions must be informed by the beginning of the month in order to avoid paying that month's tuition.
Also, costumes are ordered in October/November and the parents will be responsible for full balance on costumes after that time.
CLOSINGS DUE TO WEATHER: Dance Expressions will close with inclement weather.

Attendance: Good attendance is imperative. Please keep absences to a minimum. It is very difficult for any class to be held behind because of someone with excessive absences. Please make every effort to be at every class!
STUDIO RULES: Parents are responsible for their children's behavior while at the studio. No student should be at the studio longer than 10 minutes before or after class without adult supervision. No children to be in Mrs. Angie's office without permission. There is to be NO FOOD OR DRINK in the DANCE ROOM/studio area. Exception of water bottles for dancers.
