PAYMENT AND CANCELLATION POLICIES

In order to eliminate traffic at the main desk and to promote social distancing, we will be requiring all clients sign up for autopay for the 2020/2021 dance season. Tuition will be posted in the parent portal by the 15th of each month preceding the due date of the 1st of the coming month. Anyone wishing to pay by a method other than the card on file for autopay will be able to do so starting on the 15th when fees are posted, up to the last day of the month preceding the tuition due date of the 1st of upcoming month. On the 1st of each month autopay will be run for all fees not previously paid in the parent portal by another method.

Students may drop a class at any time. Once tuition has been paid for the month, a refund will NOT be given; therefore, students may finish the month of classes that have been paid for. In order to cancel classes without further charges made to your account, you MUST email us prior to the 1st of the next month.


POTENTIAL NEED TO PROVIDE CLASSES VIRTUALLY

Should Starz Dance Galaxy be required to cease in person learning temporarily due to Covid-19 or any other public health issue whether mandated by local, state or national officials we will provide virtual classes via Zoom or some similar platform. Classes may not be on the same day/time as in person classes. Tuition will remain in force during any period of distance learning. The written cancellation policy will remain in force as described above.

RELEASE OF LIABILITY -- READ BEFORE AGREEING

In consideration of being allowed to participate in any way in the Starz Dance Galaxy program, its related events and activities, I acknowledge, appreciate, and agree that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. The risk of exposure to infectious diseases may be increased by participation in group activities, including Covid-19, Influenza, MRSA and others. I am aware of this risk; and,

3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation; and,

4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself and my child from participation and bring such to the attention of the Company immediately; and,

5. I, for myself, my child, and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS J&J Dance LLC dba Starz Dance Galaxy, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT , FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child's involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law. I understand and agree that no oral or written representations can or will alter the contents of this document. I agree that this agreement shall be governed by the laws of the State of Florida.
