WAIVER AND RELEASE: I have chosen to have my child participate in dance and acrobatics given at Acro-Dance Company and City Soccer LLC; BAJ & CDR Holdings LLC. I acknowledge that I fully understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Acro-Dance Company, it's not in any way responsible for making such a determination.

In consideration of my child’s enrollment in any dance and acrobatic instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Acro-Dance Company and City Soccer Company and City Soccer LLC; BAJ & CDR Holdings LLC. It's Assistants, Officers, Directors and Teachers from all claims, cost, liabilities, expenses or judgments, including attorneys fees and court costs for any occurrences in connection with any dance or acro instruction. I assume all risks to my child in connection with any instruction and further release Acro-Dance Company and City Soccer Company and City Soccer LLC; BAJ & CDR Holdings LLC and it’s owners, staff and faculty from liability for any injuries sustained by my child while he or she is enrolled in any dance or Acro instruction program on the premises or anywhere we participate as representatives of Acro-Dance Company; Including all risks reasonably connected with such activities whether foreseen or unforeseen.

I understand that Acro-Dance Company and City Soccer Company and City Soccer LLC; BAJ & CDR Holdings LLC is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio, including but not limited to the parking areas and property boundary and that Acro-Dance Company will only be supervising my child when he or she is participating in scheduled activities, programs or instructions.

ACCOMPANYING PARENTS AND GUARDIANS RELEASE: I understand that Acro-Dance Company and City Soccer Company and City Soccer LLC; BAJ & CDR Holdings LLC is not responsible for any risks in the common areas and areas surrounding the studio facilities, including but not limited to the parking areas and property boundary and that Acro-Dance Company will only be supervising my child.

MEDICAL RELEASE: I give my permission to Acro-Dance Company, owners, staff and/or faculty to call 911 or take my child to a medical/dental facility, if necessary. In case of emergency if none of the contacts on Files from my child can be reached, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. Camp may be given under whatever conditions are necessary to preserve the life, limb or well-being of my dependent. I accept full responsibility for all costs of said medical care and any medical care or emergency treatment. I hereby waive claims whatsoever in connection with such Medical treatments.

PERSONAL PROPERTY RELEASE: I understand that Acro-Dance Company is not responsible for personal property that is lost, damaged or stolen while I or my child is in property premises.

PHOTO / IMAGE RELEASE: I authorize and agree that Acro-Dance Company may take and use photographs, videos or likenesses of me or my child has needed for each record-keeping, recital programs, advertising and / or public relations projects and that I have no right to the same and will not be compensated for the same.

My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement and to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.

I FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND I HAVE READ THE SAME PRIOR TO SIGNING.

* Tuition NOT PAID by the second class meeting and absence of your child for the first two weeks will result in your child losing his/her class position (AUTOMATIC DROP). When ADC does an Automatic Drop, it does not relieve you of the financial responsibility already incurred. You are still responsible for payment through the date of Automatic Drop. If your child has attended all month however your account is not paid in full your child will also loose their class position for the next month. In this case you are still required to pay for the full months tuition in which your child attended. Timely tuition is the only way to secure a class position.
* If you are no longer attending ADC and have a outstanding balance, your account will be sent to our collections agency if your balance is not paid. You will be charged an extra $25.00 fee if you are sent to our collections agency.
* You are required to pay for all classes up through the day you have given ADC written notification of withdrawal or as of ADC Automatic Drop date. Proof of notification is your responsibility. Please don’t rely on communicating verbally to the coaches or even the office staff. Request a written acknowledgement of your communication.
* Tuition is considered paid on the date received by our office staff, not the date of the check. ADC clears, posts and deposits our checks daily. Payments mailed must be received prior to the automatic payment date to receive any discounts. Please check at the office for these dates.
* There is a $25.00 charge for the first time a check is returned to us and a $40.00 charge for the second time. ADC will not redeposit your check. The only remedy for this oversight is CASH, or Charge, and must be paid by your child’s next class meeting.
* All students are required to pay an annual $35.00 membership fee. This fee is paid at the time of enrollment and then again 12 months from when you first began. This charge is non-transferable and non-refundable.
* Late pick-up charge: There will be a fee applied to your bill of $5.00 per 15 minutes that you are late for pick-up.
* Your child may make up a missed class (see Make Up Policy); however, missed classes can not be transferred to future months in place of tuition. There are no fee adjustments for missed classes.
* Absolutely NO REFUNDS are given. All gym credits given to a customer are non transferable between different families nor refundable.
* The gym will be closed for posted holidays throughout the year. Class fees are not prorated when the gym closes due to holidays because on a yearly basis, the months with 5 classes make up for the holidays.
