I will not hold Pointe Counterpointe Dance Center or anyone associated with the center responsible for injury, or lost/stolen items. I agree to pay tuition by the 1st of every month and understand that a $20 late fee will be added to my account if tuition is not paid by the 10th of each month.
I give permission for Pointe Counterpointe Dance Center to use my child's photograph or video for any advertising or publications.

By creating your family account through DSP (Dance Studio-Pro) during the on-line registration process, you agree to the following:
I agree to fill out and sign the Covid-19 Health Screening form and promise to report any symptoms, exposure, etc. ASAP.

I agree to allow the instructor to use safe, hands-on spotting techniques to ensure proper technique and to avoid injury.
I give permission for Pointe Counterpointe Dance Center to use my child's photograph or video for any advertising or publications.
